AFFIDAVIT OF WILLIAM N. COLBURN (CONTENTIONS SAPL-7 AND SAPL-33)
I, William N. Colburn, being on oath, depose.and say as follows:
1.
I am the Coordinator of Emergency Services, Division of Human Services, Department of Health and Human Servicer.
2.
Regarding the reception and registration of evacuees at Host Community Reception Centers, the Department of Health and Human Services (DHHS) is responsible for establishing and administering these centers.
Each center will be served by DHHS Emergency Services Units (ESU's),
consisting of Department personnel.

t of volunteers who will provide evacuee registration, a
message exchange'and locating service, information/ recreation, mass care referral, and student pick-up coordination.
3.
In addition to ESU core staff, there are approximately 550 Department employees in twelve field offices who provide a potential pool for staffing these centers.
In addition, there is an additional potential pool of over 300 Department employees stationed at State Offices, Concord.
Volunteers from the host and adjacent communities would also be accepted.
The attached chart illustrates projected allocation of only Department field staff to the four centers.
The total projected number of staff is considered to be adequate to serve the anticipated number of evacuees.
