1.0 REFERENCES
COPY 2.0 GENERAL PPRV 2.1 PURPOSE The purpose of this instruction is to outline the use of a rejectable NDE Report to document minimum wall violations and arc strike repairs.
2.2 SCOPE This instruction covers the documentation of minimum wall violations and arc strike repairs only.
It does not cover the removal of starts and stops and slag, defects found upon completion of the weldment at the time of final HDE, NDE performed on non-ASME safety related items,
" info" NDE, RT and UT examinations, and unsatisfactory UT, PT, and MT inspections.
3.0 INSTRUCTION 3.1 REPORTING When it has been determined that there is a minimum wall violation condition or an arc strike repair, it shall be reported and control-led using the following procedure:
a.
Document condition on a rejectable NDE report (Exhibit 1).
b.
Place a hold tag on the item and note the rejectable NDE number on the hold tag.
c.
Document the NDE rerort number and related information on the
" Rejectable NDE Report" Log" (Exhibit 2) which will.be on file with the area lead Inspector.
d.
Submit white copy of the report to welding engineering.ind file the yellow copy with the QC Supervisor. The pink copy shall be filed with the rejectable NDE Report Log.
NOTE Welding Engineering will evaluate the report and either issue an RPS to repair (for completed or partially completed welds), add additional opera-

tions to the Weld Data Card (for joints where welding has not been initialed), or generate an j
NCR for dispositioning by Mechanical Engineering.

If a RPS or a NCR is generated by Welding Engineering, the rejectable NDE report shall be closed out by documenting the RPS/NCR number, initials and date on the pink copy (which is.
on file with the rejectable NDE report log) and the " Reject-able NDE Report Log".
f.
If, additional steps have been added to the " Weld Data Card",
then it shall be so noted on the' pink copy of the rejectable NDE report and the " Rejectable NDE Report Log" closed out.
g.
The hold tag can now be removed by QC (prior to start of work).
NOTE Work shall start only after 4

additional steps have been added to the WDC, a RPS has been issued or a NCR has been dispositioned, "Use-As-Is".
h.
As each sheet of the " Rejectable NDE Report f.og" is completely filled out, the completed log shall be forwarded to the QC Vault for storage.
