Pursuant to the U.S. Atoaic t'nergy Con =innion regulations entitled "Standara f or Prot ection Against Radiation", 10CFR Part 20.405(a), we ac rc hy repor t to yeu an expcsule of an individual to radiatf or. in excess of that dsc specified iu 10CFR Pert 20.101(b) (1).

The radiatien doce rate in the arca, in which the individual was working i

wien the applicable exposure limit was exceeded, was six roentgens per hour.

The presence of rad.iation dose rates of the above mentioned magnitude in due to the formation of an adherent corrosion film on the inside curfaces of our riain coolar t system, which is not readily removable.

In thic instance the individual made three entrie-into the area cith no dosimeter check until after the third entry.

After the third entiy the individual's douimoter (0-1000mr) was chect:ed and iound tc. he off scale.

imposes a severe pennity on our overall operation.

Thic inc3dcnt was precipitated by a violation of Health Physics procedureu due to gross negligence on the part of a Health Physicc Supervicor.

As a

-recult of thin, the supervisor responsible van given a tuo week suspension vithout pay.

It is felt that the effects of this suspensjon will cerve to nahe all personnel more keen 3y avare of their responsibilities and will hc]p i
