Putsuant f,o t he ti piirement s of lechnical Specificat ions, Sect ion 0.9.4.A(5), the following is repor ted as 1,icensee livent Report 50 29/80-8.

On.fanuary 19, 1980, with the Plant in a Cold Shutdmm Cotxtition, the tine delay relays which operate the Accirwolator Nitrogen valves wre replaced wi th nn irgroved rv.stel.

Following re;11accrent, a test circuit va'; used to veri fy tirwer operatio t ard all t imrs Gru.tloned properly.

i,ater in tin week during, r.urwillarn* test inc. of t he cot ere ci rcuit ' the tirvira fnited to l

ope t a t.e.

Invest igation vi to why the tirwn s did not operat e revealed thsit the s c l ay, we r" po i n t ity <ensit ive al though them was no indicat ion of this in tic instru:.t ion lit eratut e or en the relay itsel f.

11v.: siguni leads were reversed an d t h" t. im rs wo rked p r ope rly.

The relays which had teen replaced wem irrediately checktwf and niso found

'to be pointity sensitive.

These relays hnj been installed in October,1979 nnd pinred in service Nove*r 5,1979.

The relays in service prior to Novembe r IM9 wre not poln: i ty sensi tive.

The inatn) Int ic+ doctrent at icn for the October replateiw?nt was checked to d e t e min e low t he po l a r i t y o f the ittpu t leads were connected.

Ihe as built schenatics doctrent:ttion is conflicting relative to tJte polarity orientation.

Ib Alth<y.igh the viring connection in Noverber cminot be veri fie41 with 100%

certninty, there is a prehabilit y thnt t he poln r i ty o f the si gnal lend '.<as reversed.

If, irmn furt her investination it is shown that the polarity wac in frict rever<cd, t W ucci.rulator systerr wuuld not have functioned although the roonthly mirveillance test ing would show proper timr operation.

Tiw ent i re ci rcui t will be rcter.ted to veri fy proper o;x. ration prior to j

starttp and caut ionarv st eps n&ied to approprinte doctanent s as to the polarity l

sensitivity of this equipment.
