No. 2 Main Steam Line Monitor Inoperable for Greater Than 7 Days (Proposed T.S. 3.3.3.1, Table 3.3-4 Item 2(c))

In accordance with our Proposed Change No. 228 to Technical Specification 3.3.3.1, Table 3.3-4, Item 2(c) and Technical Specification 6.9.6.k, the attached Special Report outlining the actions taken, the cause of the inoperability and the plans and schedule for restoring the instruments to operable status is hereby submitted.

B&CKGgoMND Proposed Change 228 to the Technical Specifications has been formulated and submitted for review and approval by the NRC

~

on 7/7/89.- The change will add the Main Steam Line Area Radiation Monitors (MSLARMs) and Primary Vent Stack noble gas monitor to Technical Specification 3.3.3.1 and will address the actions to be taken if the monitors are not operable.

These~ actions include initiating a preplanned alternate monitoring method'and restoring the channel (s) to operable  !

status within seven days or generating a Special Report i within fourteen days outlining the actions taken, cause of the inoperability and the plans and schedule to restore the system to operable' status.

During the time period required for the NRC to review and j approve-the proposed Technical Specification change the plant j will adhere to the proposed requirements.

On 7/23/89, No. 2 Main Steam Line Area Radiation Monitor l (MSLARM) was declared inoperable because of a high source  ;

check reading. This condition was found during the performance of a routine surveillance conducted by Radiation Protection Department personnel in accordance with procedure q OP-4816, " Functional' Test and Alarm Settings of the Area '!

ACTION TAKEN .

1 Troubleshooting of the channel was initiated by Instruments and Controls Department personnel. Following the i replacement of detector's cable and an instrumentation.

_CAUSE OF Tile INOPE_RABJJdTJ The root cause of the inoperability was attributed to i equipment deterioration resulting from environmental conditions. The monitor is mounted in an area that is subject to temperature and humidity fluctuations. The MSLARMs' installation, environmental conditions and performance remains under evaluation as part of the normal surveillance and maintenance program.
