REPORT OF INTERVIEW WiTH AUDREY SUE PEREA PEREA, Supervisor, Tritium Room, Innovative Weaponry, Inc. (IWI) of Nevada, was interviewed at the offices of IWI, Albuquerque, New Mexico, on October 20, 1995, by Nuclear Regulatory Commission (NRC) Senior Investigator Virginia Van Cleave and Investigator Dennis Boal. PEREA related the following information in substance:
PEREA said she is the supervisor of the tritium room and has held this position for approximately 1 year and has worked at IWI a total of 3 1/2 years. PEREA said her duties consisted of receiving the tritium, keeping track of the tritium, filling out a daily inventory sheet, sorting the tritium by brightness, placing the tritium into tubes, and inserting the tubes into weapon sights.
PEREA said the raw tritium is kept in the tritium room in a safe to which she has the combination. PEREA said when a bulk shipment of tritium is received, she manually counts the tritium tubes and " breaks" them into bags of 100.
PEREA said she then places the bags of 100 into larger bags containing 1000 to 2000 tubes of tritium sorted by color and size. PEREA said she notifies David M. GREGOR, IWI President, if there is a difference in the count. PEREA said at the end of each week, she provides the front office with a copy of the tritium inventory. PEREA said she makes up a board of tritium by matching two i tubes for brightness in the darkroom and then placing them on tape on the ;
board.
PEREA said the inventory sheets she maintained combined the count for the i South African [Lumitech] and Canadian [ SRB Technologies] tritium. She 1 initially said they did not use the South African tritium because they still '
had Canadian tritium. PEREA said she did have tubes made up containing the South African tritium. l PEREA said her latest inventory showed the current South African red tritium count was 3900 raw, 392 on the boards and 759 in the bags. Using a calculator, PEREA said the total tritium count, including both South African and Canadian, was 69,119 on October 13, 1995. PEREA said she did not believe they were short any South African tritium. PEREA said the
" green tritium 250" from South Africa was brighter and used in front sights.
PEREA said IW1 was using green tubes that were previously assembled although she recalled making about 400 green front sights with South African tritium and giving them to Ray D. WYRICK, IWI Quality Control Supervisor.
INVESTIGATOR'S NOTE: Several unsuccessful attempts were made to match !
the physical inventory count of the South African tritium with the l receipt documents and stock on hand. The investigators and PEREA, with I additional IWI assistance, were unable to determine the South African tritium inventory on this date.

PEREA said she is only responsible for the raw tritium. PEREA recalled that 4
Mark AGER [NFI] from Ramrod Company, South Africa, brought a supply of tritium to IWI sometime in May 1995. PEREA said AGER had previously brought a sample i of the South African tritium already inserted into tubes. PEREA added the records showed 721 tritium tubes from the first South African batch were dead because they had not been " cured."

' PEREA said she was directed by WYRICK to make sights using the South African red tritium. PEREA said WYRICK sent her a paper that detailed the sights
! requiring red tritium and a magnetic board with the sights mounted that were i to have red tritium inserted into them. PEREA said the red tritium was
- inserted into the sights, and the magnetic plate was then returned to WYRICK.
1 PEREA said both Patricia WILSON, IWI Executive Vice President, and GREGOR told j her it was all right to use the South African red tritium.
