REPORT OF INQUIRY

SUBJECT:

NORTHEAST NUCLEAR ENERGY CO., HILLSTONE POINT 1 AND 2:

UNQUALIFIED QUALITY ASSURANCE MANAGER WORKING FOR CROUSE NUCLEAR TESTING REPORT NUMBER: 1-82-040 DATE CLOSED: January 26, 1983 in April 1982, Mr. Eric JOHNSON, NRC Region IV, advised the reporting Investi-gator that he had received an allegation from an individual (confidential identity) regarding the above captioned subject. JOHNSON indicated that his source of information did not have first hand information regarding the allegation; however, the source identified a Mr, Charles HELLIER, General Manager of the Nondestructive Test Engineering Division, Hartford Steam Boiler Inspection and Insurance Company, Essex, CN., as being willing and able to provide first hand information relating to the allegation, JOHNSON said that the source stated essentially that ' for Crouse Nuclear Testing at one of the Hillstone Point Nuclear Power Stations, was sus-pected of forgingMDT Level !!! certification papers and not qualified fo rM po sition, According to JOHNSON, the source said that had attended HELLIER's school for Level Ill NDT certification, had flunked out o the school and took a blank graduation diploma withMwhich Emay have used for Level 111 certification. JOHNSON did not provide any additional pertinent information.

The guidelines for personnel qualifications and certification in nondestructive testing is set forth in the " Recommended Practice No, SNT-TC-1A" (June 1980 edition) which was developed by the American Society for nondestructive testing -

to aid employers in recognizing the essential factors to be considered in qualifying employees engaged in any of the test methods listed in the guide-line. The guideline states that certification of all levels of NDT personnel is the responsibility of the employer; all levels of NDT personnel should be recertified at least once every three years; and the employer's certification shall be deemed revoked when employment is terminated.

Mr. Charles HELLIER, Supra, was interviewed by the reporting investigator at HELLIER's place of business on December 8,198 HELLIER said that in about 1975 M came to him stating that wanted to attend a Level III training course t his (HELLIER's) sc . HELLIER said thatMIhad not taken any diplomas that could be used for cer-tification purposes stating that his school, neither during the time period

in question, nor presently, certifies individuals for the NDT methods and that I

certification is the responsiblity of the employer. A result of interview with Mr. HELLIER setting forth additional details is attachment (1) to this report.

Inquiries at the Millstone Nuclear po Facilities by the NRC Senior Resident inspector (SRI) determined that The SRI further stated that during the time of employment, the Crouse QA/QC program had not been accepted by the licensee and a QA/QC record review did not surface any inspection packages bearing M s!sionature. The SRI stated that the' current Crouse site manager reported thatgwas never certified at Millstone by Crouse as a Quality Control Inspector and the SRI reported that'g,ltraining folder was no longer maintained at the Millstone sites.
