4 October 1995 The reactor operated continuously in October with the following exceptions: seven shutdowns for scheduled maintenance and refueling or flux trap changes. There were no unscheduled shutdowns this month.

Major maintenance items for the month included: installing a new Gamma-Metrics source range monitor; replacing the dual scram trip unit for intermediate range channel #2; biennial change-out of control blade offset 'C'; installing a new rhodium detector in the G-4 position; removing the old source range monitor detector and drywell; removing secondary pool heat exchanger bypass valve S-2 and replacing it with a 6 inch bypass line; replacing the shear pin on the outer personnel airlock door.

10/16/95 Refueled - removed core 95-43, loaded core 95-44. Installed new Gamma Metrics source range monitor; replaced dual scram trip unit for intermediate range channel

10/30/95 Refueled - removed core 95-45, loaded core 95-46. CONFIRMATORY ACTION LETTER

Dear Dr. Rhyne:

This letter refers to NRC concerns with the control of radioactive . topaz at

" your facility identified by Mr. R. Marsh during an investigation on a

September 19 and 20, 1995.

Pursuant to Mr. Steve Gunn's letter to Mr. Marsh dated September 20, 1995, as amended on September 21, 1995, it is our understanding that you will:

(1) padlock the inner door to the dark room and place two keys to the room under the control of Mr. John Ernst or his designee and Mr. Matt Sanford or his designee, (2) until further notice, have a University of Missouri Research Reactor (MURR) employee present in a supervisory role when any work is performed in this space,

" l (3) conduct an audit of the material contained in the room under the l direction of Mr. Jim Schuh,

  • j' (4) ensure no material is transferred into or out of this room until the inventory has been reconciled under the oversight of Clarence Jett of the University of Missouri Internal Auditing Department, and (5) once the inventory is complete and un'til further notice, ensure a MURR employee supervises all material transfers into and out of the dark room.

Pursuant to Section 182 of the Atomic Energy Act, 42 U.S.C. 2232, you are required to:

1) Notify me immediately if your understanding differs from that set forth above; i

schedule in advance of the change; and 3) j Notify me in writing when you have completed the actions addressed in this Confirmatory Action Letter.

Issuance order of this Confirmatory Action Letter does not preclude issuance of an formalizin of the licensee;g the above commitments or requiring other actions on the part nor does it preclude the NRC from taking enforcement action for violations of NRC requirements that may have prompted the issuance of this letter. In addition, failure to take the actions addressed in this Confirmatory Action Letter may result in enforcement action.
