& 2 Verification of Concerns EFFECTIVE DATE 02/24/92 1.O PURPOSE This instruction establish'is how SPEAKOUT will perform documented follow-up verifications on response actions to concern recommendations and how SPEAKOUT will disseminate information to Quality Assurance regarding actions taken for concerns for follow-up and effectiveness evaluation during quality verification activities.

2.0 SCOPE 1

This instruction applies to response actions to concern recommendations which are not related to individual disciplinary actions.

Items which could cause a

perception that the integrity of the SPEAKOUT program has not been maintained will not be disseminated to Quality Assurance.

3.0 DEFINITIONS Refer to Section IV, Appendix C, Definitions.

4 5.o RESPONSIBILITIES 5.1 The

Manager, Quality Performance /SPEAKOUT is responsible for the maintenance and implementation of this instruction.

5.2 The Manager, Quality Performance /SPEAKOUT is responsible for ensuring that follow-up verifications on response actions to concern recommendations are performed and documented.

6.0 REQUIREMENTS 6.1 The

Manager, Quality Performance /SPEAKOUT will assign an individual to verify that a response action has been completed.

2 EFFECTIVE DATE 02/24/92 6.2 The assigned individual will verify that objective evidence of the completion of the action exists.

6.3 The individual Will provide docusentation of the verification for the

Manager, Quality Performance /SPEAKOUT's review.

6.4 The documentated verification will be filed with 1

the originating SPEAKOUT concern.

6.5 The

Manager, Quality Performance /SPEAKOUT will review investigation reports and disseminate appropriate information to Quality Assurance regarding actions taken for concerns for follow-up and effectiveness evaluation during quality verification activities.
