1.0 PURPOSE This instruction delineates how concerns will be filed and tracked throughout the SPEAKOUT process.

2.0 SCOPE This instruction applies to the tracking of concerns and specifically the receiving, documenting, opening, computer.

30 and inputting, filing, requesting additional information, 60 day letters, routing, storing, mailing, tracking and closing of a concern.

3.0 DEFINITIONS Refer to Section IV, Appendix C, Definitions.

5.0 RESPONSIBILITIES The Manager, SPEAKOUT Progra:, has the responsibility 5.1 for the maintenance and ing:.ementation of this instruction.

The Manager, SPEAKOUT Program, is responsible for the 5.2 implementation of this instruction and will monitor implementation of this instruction periodically.

The SPEAKOUT Interviewer / Investigator is responsible for 5.3 implementation of any portion of this instruction that is applicable to the receiving, filing and tracking of.a concern within SPEAKOUT and specifically receiving and documenting concerns; opening concerns; computer l

6.0 REQUIREMENTS 6.1 Upon receiving a concern by telephone, walk-in or scheduled interview, the concern will be documented on a-SPEAKOUT Concern Record.

A mail-in concern is received-on a SPEAKOUT Concern Report.

The concern is then transferred onto a SPEAKOUT Concern Record.

6.2 Part one (top part with the concernee's name, address, and concern number) of the Concern Record will be manually logged in the SPEAKOUT Interviewer's Weekly Traffic Report and then retained by the Interviewer / Investigator.

6.2.1 Omitted.

6.3 Information received from the concerned will be analyzed by the Interviewer / Investigator and briefly summarized and typed on the bottom portion of the Concern Record.

This summary will reflect the actual concern of the concerned party.

A transcription of the tape recording, if any, is typed and attached to the concern record.

6.4 After classification of the concern is made, the following is accomplished:

6.4.2 The SPEAKOUT Interviewer / Investigator will create a Concern File Folder.

A copy of the original Concern Record, and all related documentation will be placed in this concern File Folder.

A copy of Class 1 concerns with tape transcriptions will be provided to the assigned SPEAKOUT Investigator.

Class 2 through 5 will be forwarded to the cognizant department for investigation._ See paragraph 6.10 below for disposition of Class 2 through-5 concerns.

6.4.3 The following information will be inputted into the SPEAKOUT computer by the Interviewer / Investigator.

Date assigned SPEAKOUT Program, will review Class 1 6.5 The Manager, concerns for subject, content, priority, difficulty of investigation, need for additional information and assign the concern to a SPEAKOUT Investigator for investigation.

6.5.1 If a determination is made by the Manager, SPEAKOUT Program, or the assigned Investigator, that additional information is required from the concerned party, a memorandum describing specific information required, will be drafted and approved, and forwarded to the SPEAKOUT Interviewer / Investigator for mailing to the concerned party.

The SPEAKOUT Interviewer / Investigator will 6.5.2 mail the letter and track the concerned party's response on the Information Log, ensuring that the concerned party's anonymity remains intact.

When an answer from the concerned party is 6.5.3 received, the log will be annotated to reflect this.

The letter is forwarded to the Manager, SPEAKOUT Program.

The original letter is filed.

If no answer is received in 30 days from the 6.5.4 concerned party, the Manager, SPEAKOUT Program, will be notified by memorandum from the SPEAKOUT Interviewer / Investigator requesting disposition of the concern.

6.8 When the Investigation Report is approved by the Manager, SPEAKOUT Program, the SPEAKOUT Investigator, will prepare a draft Response Letter.

(NOTE:

Section 6.8 applies only if the concernee requested a response and provided an address.)

The SPEAKOUT Interviewer / Investigator will 6.S.1 prepare a draft Response Letter which will consist of a letter that is responsive to the concern.

6.8.2 The draft Response Letter is then forwarded to the responsible Investigator to proofread and when complete to the Manager, SPEAKOUT Program.

6.8.3 The Manager, SPEAKOUT Program, will review the draft Response Letter for those items listed in paragraph 6.8.1.

The Investigation Report and Response Letter are then retained until the SPEAKOUT Review Committee meeting.

The draft Response Letter and Investigation 6.8.4 Report are stamped "For Review" and transmitted to the SPEAKOUT Review Committee after review and approval by the Manager, SPEAKOUT Program if applicable.
