REVIEW OF CLASS 1 INVESTIGATION REPORTS

1.0 PURPOSE This instruction delineates how Investigation Reports will be reviewed for potential quality impact and a requirement for root'cause, generic implication or corrective action analysis.

2.0 SCOPE This instruction applies to Class 1 Investigation Reports.

for the implementation, maintenance, and updating of this instruction.

5.2 The Manager, SPEAKOUT Program, has the responsibility for reviewing reports to determine if a potential

. quality impact exists.

5.3 The Manager, SPEAKOUT Program, is responsible for assuring that potential quality impacts are forwarded to the proper organization for evaluation.

ErrECTIVE REVIEW OF CLASS 1 IINESTIGATION REPORTS DAE Each Investigation Report will be assessed for potential 6.1 and whether root cause, generic quality impact, implications or corrective action analysis may be required.

When a potential for any of the items in 6.1 is 6.2 identified, SPEAKOUT will generate a discrepancy document or forward the information to the appropriate organization.

7.0 DOCUMINTATION All documentation will be filed in the respective SPEAKOUT Concern Investigation Report File Folder.
