1.0 PURPOSE This instruction delineates how Investigation Reports will be reviewed for potential quality impact and a requirement for root'cause, generic implication or corrective action analysis.

2.0 SCOPE This instruction applies to Class 1 Investigation Reports.  !

5.3 The Manager, SPEAKOUT Program, is responsible for assuring that potential quality impacts are forwarded to  ;

the proper organization for evaluation.

SPEAKOUT PROGRAM INSTRUCTION SPI-12 p ErrECTIVE REVIEW OF CLASS 1 IINESTIGATION REPORTS DAE 6.1 Each Investigation Report will be assessed for potential quality impact, and whether root cause, generic implications or corrective action analysis may be required.

6.2 When a potential for any of the items in 6.1 is identified, SPEAKOUT will generate a discrepancy document or forward the information to the appropriate organization.
