1.0 PURPOSE This instruction delineates how SPEAKOUT Response Letters are.

written, approved and forwarded to individuals expressing a concern to SPEAKOUT.

2.0 SCOPE This instruction applies to all Response Letters.

3.0 DEFINITIONS Refer to Section IV, Appendix C, Definitions.

5.0 RESPONSIBILITIES t

5.1 The Manager, SPEAKOUT Program, has the responsibility for the maintenance, implementation and updating of this instruction and keeping the SPEAKOUT Review Committee advised of any significant changes.

5.2 The Manager, SPEAKOUT Program, and the SPEAKOUT Review Committee are responsible for the review of all final Class 1 Response Letters, if a response is required.

4, and 5 concerns assigned to their organization.

5.4 The Manager, SPEAKOUT Program, or designee is responsible for the review and signing of all Class I, 2,

3, 4, and 5 Response Letters provided to concerned individuals.

5.5 The Manager, SPEAKOUT Program, is responsible for providing the SPEAKOUT Interviewer / Investigator a copy of the Class 1 SPEAKOUT Investigation Report to draft a Response Letter to accompany the Investigation Report to the SPEAKOUT Review Committee when applicable.

5.6 The SPEAKOUT Investigator or designee is responsible for drafting a Response Letter, if required, for each Class 1 concern Investigation Report.

Additionally, the SPEAKOUT Investigator is responsible for transferring Responses to the SPEAKOUT letterhead s ationery when applicable, with a signature block for the Manager, SPEAKOUT Program, and mailing all Response Letters to the concerned individuals.

5.7 Omitted.

6.0 REQUIREMENTS 6.1 When Class 1 Response Letters are approved by the SPEAKOUT Review Committee, the letter will be forwarded to the Interviewer / Investigator for mailing.

6.2 Omitted.

6.3 The Manager, SPEAKOUT Program, reviews all Class 2, 3,

4 and 5 Responses provided by cognizant organizations for content, clarity and accuracy.

6.4 The SPEAKOUT Investigator or designee will transfer the Response to SPEAKOUT letterhead stationery with a signature block for the Manager, SPEAKOUT-Program, or designee for signature, if applicable.
