CLASS 2, 3, 4 AND 5 CONCERN PROCESS EFFECTIVE DATE 02-22-93 1.O PURPOSE This instruction delineates how Class 2, 3, 4, and 5 concerns will be processed.

2.0 SCOPE This instruction applies to the receiving, classification, keyword code assignment, routing, investigation, the need for additional information and responses to Class 2, 3, 4, and 5 concerns.

5.O RESPONSIBILITIES i l

i 5.1 The Manager, Quality Performance /SPEAKOUT, has the overall responsibility for the implementation, maintenance and updating of this instruction. The j Manager, Quality. Performance /SPEAKOUT, is responsible for signing Response Letters to Class, 2, 3, 4, and 5 concerns, when required, and ensuring that they are responsive to the concerns.

5.2 The Manager, Quality Performance /SPEAKOUT, is responsible for the implementation of this instruction in its entirety. The Manager, Quality Performance /SPEAKOUT, Will serve as the point of contact for - cognizant departments responding to Class 2, 3, 4, and 5 concerns.

responsible for receiving concern (s). The specific responsibilities include receiving the concerns by mail-in, walk-in, telephone or scheduled interviews; documenting the concern on a SPEAKOUT Concern Record; preparing a transcript of the concern; and requesting additional information from the concernee, if necessary.  ;

5.4 Cognizant departments who receive a Class 2, 3, 4, or 5 concern from SPEAKOUT are responsible for ,

conducting a thorough investigation into the concern; evaluating the results of the investigation; making . recommendations, if any, preparing a response to the concern; and forwarding their response to SPEAKOUT. NOTE: In those cases where a company policy, procedure or existing law already addresses the concern, the response will not require any extensive investigation, documentation or detailed response by the cognizant department.

5.5 The Manager, Quality Performance /SPEAKOUT, will review these responses for approval. Should the Manager, Quality Performance /SPEAKOUT, disapprove a response as written, any suggested changes to the letter will be forwarded to the cognizant department for their approval and incorporation '

into the response. The approved response will be .

transferred to SPEAKOUT letterhead staticnery, when ',

applicable. A copy of the original response from '

the cognizant organization (s) or an initialed copy (by the cognizant organization) of the response will be maintained in the Concern File Folder.

6.0 REQUIREMENTS .

6.1 When a SPEAKOUT concern is received it is c2assified and assigned a keyword code by the Manager, Quality Performance /SPEAKOUT, as a Class 2, 3, 4, or 5 concern.

6.2 The Manager, Quality Performance /SPEAKOUT, will review and assign the concern to a cognizant -

department.

3, 4 AND 5 CONCERN PROCESS 3 4 EFFECTIVE DATE 02-22-93 6.2.1 The Manager, Qua1ity Performance /SPEAKOUT, will review the transcript and Concern Record to ensure that the concern is as stated.

6.2.2 If a determination is made by the Manager, Quality Performance /SPEAKOUT, that additional information is required to successfully investigate the concern, he/she will request specific information be obtained, if possible.

6.3 A letter cf transmittal signed by the Manager, Quality Performance /SPEAKOUT, the concern statement, and the transcript are transmitted to the cognizant organization.

6.4 The assigned cognizant department is responsible for conducting a thorough investigation into any concern received and responding to SPEAKOUT.

6.4.1 All investigations will be resolved as to who, what, when, why and how, if possible, by the cognizant department.

6.5 Should the cognizant department determine that additional information is required to satisfy paragraph 6.4.1 above, a request will be forwarded 3 to the Manager, Quality Performance /SPEAKOUT, requesting the specific information required, t 6.5.1 SPEAKOUT will

obtain the additional information, if practicable.

6.6 The Manager, Quality Perforamnce/SPEAKOUT, will review the responses for adequacy and accuracy. If any comments are necessary, the comments will be returned to the cognizant department and steps 6.4 r

and 6.5 will apply.

6.7 Upon approval by the Manager, Quality Performance /SPEAKOUT, the response will be transferred to SPEAKOUT letterhead if applicable.

7.O DOCUMENTATION 7.1 Required documentation for Class 2, 3, 4, and 5 concerns say be in computer stored form, hard copy file or microfilm file form.

7.1.1 All class 2, 3, 4, and 5 concern

' documentation shall be controlled to include controlled storage of computer, microfilm and hard file copy information.

7.1.2 Omitted.

7.1.4 The retention period of Class 2, 3, 4, and 5 concerns documentation is " Life of Plant" as determined by HL&P Information Records Management Document Type List (DTL).
