1.0 PURPOSE This instruction establishes how SPEAKOUT recommendations or suggestions will be submitted to the SPEAKOUT Review Committee and transmitted to South Texas Project Management.

2.0 SCOPE This instruction applies to recommendations or suggestions made by SPEAKOUT as a result of Investigations and forwarded to the SPEAKOUT Review Committee for approval.

3.0 DEFINITIONS

5.0 RESPONSIBILITIES 5.I The Manager, SPEAKOUT Program, is responsible for the maintenance and implementation of this instruction.

5.2 The SPEAKOUT Investigator is responsible for developing recommendations and suggestions from his/her investigation.

5.3 The SPEAKOUT Review Committee is responsible for approval of recommendation (s) or suggestion (s) to the South Texas Project Management.

6.0 REQUIREMENTS 6.1 The SPEAKOUT Investigator will determine whether recommendations or suggestions are warranted as a result i

of his/her investigation of a concern.

6.2 When warranted, recommendations or suggestions are included in the SPEAKOUT Investigation Report.

2 PM?E OF RECOMMENDATIONS / SUGGESTIONS org 09/21/90 6.3 The SPEAKOUT Review Committee determines the validity of recommendations or suggestions as part of the Investigation Report.

6.4 Recommendations which are determined to be-valid are then forwarded to the cognizant HL&P organization by the SPEAKOUT Review Committee Chairman.

6.5 The cognizant HL&P organization shall designate the individual (s) responsible for providing the response to the SPEAKOUT recommendation (s).

6.6 All responses to SPEAKOUT shall be forwarded to the Manager, SPEAKOUT Program.

6.7 All responses received by SPEAKOUT shall be reviewed by the appropriate SPEAKOUT Investigator to ensure that the response meets the intent of the recommendation.

6.8 The response is then forwarded to the Manager, SPEAKOUT Program, for final review, and approval.

6.9 Documentation of actions taken as a result of SPEAKOUT recommendations or suggestions shall be maintained in the appropriate Concern File Folder.
