Based on the results of an ?!RC inspection conducted on September 22, 1977, it appears that certain of your activities were not conducted in t

full compliance with the conditions of your license as indicated below.

Technical Specification 4. states, in part, "The Facilities Engin-eering 3 ranch (FEB) shall perfem the folicwing activities.

i (a) Heating and Ventilating System f

(1) A weekly visual inspection.

l (2) A :ronthly check of all equipment in accordance with a written checklist...

l Contrary to the above, conthly checks of the Heating and Ventilating l

systems were not perfomed during the months of February and July, 1977.

I This violation appears to be repetitive in that a violation of l

Technical Specification 4.a was identified during another inspection, (47/75-01).

10 CFR 20.202(b)(3) states, "High Radiation Area reans any

j Contrary to the above, the area inside the reactor pool above the core plate was not posted as a High Radiation Area on September 22, l

1977. A servey of this area perfomed on July 25, 1977, shcwed a contact reading of 2400 mrec/hr en the core plate. A survey conducted l

on October 10, 1977, showed a r:aximum contact reading of 2950 aren/nr and a general area reading of 265 mrea/hr three feet fren l
