REPORT 80 EVENT DESCRIPTION AND PROBABLE CONSEQUENCES h o 2 lDuring routine surveillance testing, the HPCI pump turbine tripped on overspeed l o 3 [ prior to reaching rated system flow. The HPCI system was declared inoperative. The l g ] redundant system, ADS, was operable as were the RCIC and LPCI systems. This event I o s lis not repetitive. There were_no adverse effects on the public health and safety. I I

3J 34 35 36 3/ 40 42 43 CAUSE DESCRIPTION AND CORRECTIVE ACTIONS h i O IThis occurrence was caused by an improper valve line-up procedure. The procedure I i , [ stated the HPCI trip unit oil supply valve should be "open". This valve should be l

( i,,,i [ throttled to maintain % 20 psig unit oil supply. Excessive oil pressure caused the l l j ,;3l [ turbine to trip below operating speed. The valve was throttled. The system tested j l

l i 14 l l satisfactorily. The procedure has been revised. No further action is planned.
