Conditions Prior to Occurrence:

The reactor was in cold shutdown candition during a maintenance outage.

While performing Surveillance Test Procedure 6.2.1.6.1 during the 1981 Refueling Outage an uncalibrated temperature switch was in-stalled in MS-TS-148D position. This error was discovered while

Designation of Apparent Cause of Occurrence:

The apparent cause of this occurrence was personnel error.

Analysis of Occurrence:

The function of temperature _ switch MS-TS-148D is to monitor area temperature above the main steam line and to initiate main steam line isolation upon sensing high temperature. The redundant temp-erature switches were replaced during 1981 spring refueling outage and were operating properly.

During the spring 1981 refueling outage the temperature switch MS-TS-148D was removed.

Instead of being replaced with a calibrated spare, it was replaced with a switch removed from another location which had not been calibrated.

The temperature switch removed from MS-TS-148D was tested and found to be within Technical Specification limit. This occurrence pre-sented no adverse consequences from the scandpoint of public health and safety.

Corrective Action

Temperature switch MS-TS-148i was replaced with a switch that had been recently calibrated. The personnel error in not replacing this switch with a calibrated replacement was discussed with the technicians involved. To provide the. technician with better guid-ance, the temperature switches will be color-coded so that on any operating cycle only one color switches should be installed.

Surveillance Test Procedure 6.2.1.6.1 will be revised to provide the technician with the identifica*. ion ~information on the switch that is to be removed.- These items will be completed prior to the end of this operating cycle.
