Corrective Action

1.

The snubber was repaired, tested, and reinstalled.

2.

The steam condensing lines on both RHR loops were inspected for other damage.

3.

The system was operated in the same method as caused the failure. No signi-ficant movement or unusual conditions were observed.

Action To Prevent Recccurrence 1.

The frequency of venting of the RHR system was increased from monthly to daily until an evaluation justifies a reduction in the frequency.

2.

Venting was initiated on the steam condensing line every four hours to prevent the for=ulation of a bubble in the event there is leakage.

3.

Engineering is continuing to investigate tne cause of failure and improved isolation for the condensing line.

4.

An investigation into the failure mode of the shaft is being performed.
