1 LER A'1TACHMENT - RO #1-81-30 Facility: BSEP Unit No.1 Event Date: 2-19-81 Both of these events resulted from the NO21D instrument switch number 1 actuation screw out of adjustment. After the first event the switch adjustment screw was readjusted and the instrument was recalibrated according to plant procedures and returned to service. Following the 3-17-81 cvent, an inves-tigation determined that the actuation. screw adjustment band was not sufficient to allow for operation of the switch within its calibration range, taking into-account normal instrument drift encountered by the. instrument..The switch actuation linkage and the switch actuation screw were then readjusted to allow sufficient linkage movement and reaction with the screw to permit calibration of i

the instrument closer to the center of its specified operating band. The switch was then tscalibrated and retu'rned to operation.

~ l, A review of the calibration records for this instrument shows that from 8-7-78 up to the 2-19-81 event, no switch out of adjustment or failure to operate events had occurred. This instrument is presently on a quarterly calibration and monthly functional test schedule to ensure proper maintenance and operation.

As a result of the 3-17-81 event, the N021D instrument will be calibrated weekly for the next month to ensure no further problems exist with its proper This and other similar instruments are scheduled to be replaced with operation.
