Description and Corrective Action On May 19, 1980 at approximately 2320 hours0.0269 days <br />0.644 hours <br />0.00384 weeks <br />8.8276e-4 months <br /> following the performance of routine HPCI System surveillance test number 8.5.4.1, the HPCI system was declared l inoperable. The HPCI flow controller was not operating satisfactorily. ,.

Surveillance testing was immediately initiated as required by Technical Specifications. i Investigation revealed that wiring attendant to the installation of the alternate shutdown panel for the HPCI system was interfering with normal operation.

The alternate shutdown panel wiring was isolated and the HPCI system declared -

operable at approximately 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br /> on May 20, 1980.

On May 21, 1980 at approximately 1259 hours0.0146 days <br />0.35 hours <br />0.00208 weeks <br />4.790495e-4 months <br />, the HPCI system was removed from service to repair a gland seal exhaust leak. Simultaneous with this repair I&C investigated and found a wiring error in the control circuits of the alternate shutdown panel. The seal leakage and wiring error were corrected and the HPCI system returned to service on May 22, 1980 at approximately 1437 hours0.0166 days <br />0.399 hours <br />0.00238 weeks <br />5.467785e-4 months <br />.  ;

on May 22, 1980 at 1005 hours0.0116 days <br />0.279 hours <br />0.00166 weeks <br />3.824025e-4 months <br /> during the performance of RCIC system surveillance testing, a turbine control anomaly was identified in the RCIC turbine control unit.

A priority maintenance request was initiated to investigate and resolve any l problems encountered. A controlled shutdown was also initiated since the HPCI system was known to be inoperable at the time. At approximately 1735 the same day, the anomaly with ' the RCIC turbine control system was resolved and the RCIC system returned to service.  ;

The RCIC systems control room panel speed controller, a General Electric unit, was repaired. The alternate control panel (G.E. design and Bailey manufactured) noise feedback loop which was interfering with speed indications was eliminated.

The HPCI and RCIC systems including their associated alternate shutdown panels  !

are in service and operating in their intended manner. I i
