Description

On April 1,1980 surveillance test number 8.B.7, Part B was conducted on the diesel generator ' A',

fixed dry chemical, fire protection system.

The system's Derby Release mechanism failed to operate af ter four minutes of heat lamp exposure. Past experience with the system indicated that the system normally actuated af ter forty to sixty seconds of test lamp exposure.

The problem was immediately investigated by PNPS I&C personnel. The in-vestigation revealed that a Pyrotronics, type DTF 190 Heat Actuated Device (HAD) was erratic in its operation. Since no DTF 190 HAD's were available, the unit was re-installed.

Corrective Action

A constant fire watch has been posted in the generator room since the system malfunction and the watch will continue until the unit is repaired.

A PNPS Withdrawal Requisition number 324277 was immediately generated to purchase 2-DTF 190 HAD's from Pyrotronice Incorporated and the order telephoned into the Pyrotronics office in Needham, Massachusetts. On April 17, 1980 Purchase Order number 23807 was secured from the BECo Purchasing Department.

Pyrotronics was again contacted and they informed us that we could expect delivery by May 8,1980.

Upon reception of the replacement HAD's the intermittent unit will be replaced and tested. Following successful completion of this testing, the dry chemical system will be returned to normal operation and the fire watch discontinued.
