, . ATTACHMENT TO LER 79-Ilo/03X-1 Page 1 of 1 During normal' operations , on December 14, 1979, the HPCI System Steam Supply Inboard- Isolation Valve . (23-MOV-15) closed as a result of a spurious isolation signal. This signal and the closing of 23-MOV-15 rcndered the HPCI' System inoperable at a . time period when it.was required to be operable by Technical' Specifications Appendix A, Paragraph 3.5.C. Since no confirm-Ing indication, with respect to the isolation, was available,' the isolation i

- signal was immediately reset and 23-MOV-15 was reopened to restore the HPCI System to an operable status. The re fo re , the event did not represent any significant hazard to the public health and safety.

Extensive troubleshooting of the portion of the primary containment isolation system associated with 23-MOV-15 was conducted and no cause for the spurious signal could be found nor could the condition be duplicated.

In addition, instrument surveillance of those instrument subsystems which could cause isolation of 23-MOV-15 was conducted and no cause for the isola-tion could be found.

On February 13, 1980 another spurious isolation occurred. investigation revealed a defective wire termination at one (1) thermocouple associated l with the HPCI System isolation logic. Retermination corrected the deficiency.

- No further action to prevent recurrence is requi red.

NOTE: The February 13, 1980 event was reported in LER 80-024.

Items 9, 10, 12 through 15, 17, 18, and 25 _through 27 were revised in addi tion to the attachnent.
