The signoff for returning the isolation valves to the open position on PT 3.1.12PC was signed off; however, that signoff also includes two other items requiring act' ten. All I&C PT's will be reviewed and revised as required to provide a signoff specifically for returning operated valves to their normal position.

This event will be reviewed by all I&C and Electrical personnel. ADS permissive for "LPCI pump running" would have been available from five other sources -

RHR pumps A, B, C, D and Core Spray Pump A.
