The three tanks were not properly anchored to the supporting steel beam foundation. Had a seismic event occurred, the tanks could have shifted possibly resulting .in the stoppage of fuel oil flow to the respective D/G {EK}. The root cause of this event has been attributed to inadequate design .and design review. The Day Tanks have apparently been in the as found condition since installation. A review of the historical documentation (e.g.,

prints and specifications} re~ealed that anchoring of the Day Tanks was not specified. Upon discovery of the Salem Unit 1 Day Tank design concern, Salem Unit 2's Day Tanks were inspected. The Unit 2 tanks were found to be properly anchored. The Unit 1 Day Tanks were weld anchored to the wide flange steel beam foundation on February 12, .1988 in accordance with seismic design criteria. Also, the Day Tank civil.

drawings have been updated to identify the anchor requirements.

IDENTIFICATION OF OCCURRENCE:

Diesel Generator Day Tanks Do Not Meet Seismic Criteria Event Date: 02/11/88 Report Date: 05/18/88 This report was initiated by Incident Report No.88-048.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 Reactor Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

On February 11, 1988 at 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br />, PSE&G engineering personnel identified a deficiency associated with the Salem Unit 1 Diesel Generator (D/G) Day Tanks {DC}. The three tanks were not properly anchored to the supporting steel beam foundation. Had a seismic event occurred, the tanks could have shifted possibly resulting in the stoppage of fuel oil flow to the respective D/G {EK}.

The Nuclear Regulatory Commission was notified at 1845 hours0.0214 days <br />0.513 hours <br />0.00305 weeks <br />7.020225e-4 months <br /> on February 11, 1988 in accordance with Code of Federal Regulations 10CFR 50.72(b) (2) (iii) (D).

APPARENT CAUSE OF OCCURRENCE:

.The root cause of this event has beeri attributed to inadequate design and design review.

The Day Tanks have apparently been in the as found condition since installation. A review of the historical documentation {e.g., prints and specifications) revealed that anchoring of the Day Tanks was not specified.

ANALYSIS OF OCCURRENCE:

There are three (3) D/G Day Tanks, one per D/G. They contain the supply of fuel oil for their respective D/G. If a design base seismic event occurred with the tanks not properly anchored, the Day Tanks could have shifted resulting in the possible discontinuation of fuel oil flow to the D/G's thereby rendering all three D/G's inoperable.

The D/G's are required to be available to mitigate the consequences of a loss of off-site AC power during normal and accident conditions.

Since the Day Tanks condition could have prevented the fulfillment of the D/G's from operating during a design basis event this event is reportable to the Nuclear Regulatory Commission in accordance with Code of Federal Regulations 10CFR 50.73 (a) (2) (v) (D).

CORRECTIVE ACTION:

Upon discovery of the Salem Unit 1 Day Tank design concern, Salem Unit 2's Day Tanks were inspected. The Unit 2 tanks were found to be properly anchored.

On February 12, 1988 weld anchoring of the Unit 1 Day Tanks to the wide flange steel beam foundation in accordance with seismic design criteria was completed and the D/G's were declared operable.

The PSE&G Day Tank civil drawings have been updated to address the anchor requirements.

DPR-70 DOCKET NO. 50-272 UNIT NO. 1 LICENSEE EVENT REPORT 88-001-01~ SUPPLEMENT This Supplemental Licensee Event Report is being submitted pursuant to the requirements of 10CFR 50.73(c). The Apparent Cause of Occurrence Section and the Corrective Action Section have peen updated. The original LER did not specify the apparent root cause due to continuing investigation(s) associated with the described event. This supplement identifies the apparent root cause based upon the results of the completed investigation(s). Additionally, a corrective action has been added.
