Loss of Off-Site Power Source CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100%

Unit Load 1140 MW DESCRIPTION OF OCCURRENCE:

At 2227 hours0.0258 days <br />0.619 hours <br />0.00368 weeks <br />8.473735e-4 months <br />, a ground occurred on a power supply line to #3 Unit which caused 2T60 breaker to open resulting in #2 Station Power Transformer being de-energized.

The 13 kV breakers #4-5 and #3-4 opened de-energizing #12 Station Power Transformer.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A ground in the underground cable in the power supply to #3 Unit.

ANALYSIS OF OCCURRENCE:

Upon loss of #2 Station Power Transformer and #4-5 and #3-4 13 kV Breakers, #12 Station Power Transformer was de-energized because

  1. 2-3 and #5-6 13 kV Breakers are normally opened to provide two independent circuits.

With #5-6 Breaker normally open and #4-5 Breaker opening on loss of 2T60, all power was lost to #12 Station Power Transformer.

"i.ER 80-14/03L CORRECTIVE ACTION:

A new power supply cable was installed removing the ground allowing 2T?O_to be re-energized.
