No. 15 Containment Fan Coil Unit Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

On 12-11-78, while No. 15 Containment Fan Coil Unit (CFCU) was operating in low speed, the Control Operator observed that service water flow through the unit did not meet the required 2500 gpm. No. 15 CFCU was declared inoperable at 1635 and the Action Statement for Technical Spec-

{f ication 3.6.2.3 was implemented. Performance Department was notified and rework of the service water valve controls was commenced. No. 15 CFCU was returned to service and satisfactory tests verified its operability.

The Action Statement was terminated at 2237 on 12-11-78.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was an open in the coil of the solenoid valve for 15SW57. This service water valve is a two position valve which opens partially or fully to establish the fast or slow speed service water flow rate through the CFCU. The open solenoid coil prevented 15SW57 from opening fully when 15CFCU was operated in slow speed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3 requires that with one group of containment cooling fans inoperable, restore the inoperable fan group to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

No. 15 CFCU was declared operable 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> after it was removed from service.

No. 11, 12, 13 and 14 containment fan coil units were operable throughout this occurrence.

CORRECTIVE ACTION:

The open solenoid coil was replaced and the "O ring in the solenoid valve was replaced. 15SW57 was satisfactorily tested and returned to service.
