No. 13 Steam Generator Steam Flow Channel I i.noperable CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode I Reactor Power 45%

DESCRIPTION OF OCCURRENCE:

Channel checks by the Control Operator on 11-30-78 as the unit was at low power level showed No. 13 Stearn Generator Stearn Flow Channel I reading lower than other channels. A work order was issued to Performance Department and calibration of the sensor transmitter was started at 1030.

Initial "as found" data showed a zero shift and output readings which were greater than allowed by Technical Specification. The channel was declared inoperable at 1100 and Technical Specification 3.3.1.1, Table 3.3.1, Action 7, was implemented. The channel was placed in the tripped condition and remained tripped until 1330 on 12-1-78 when the channel was declared operable and the Action Statement was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was a 100 millivolt zero shift of the transmitter caused by the shutdown and startup of the unit. This is not an uncommon occurrence with these transmitters.

ANALYSIS OF OCCURRENCE:.

Technical Specification 3.3.1.1, Action 7, requires that with the number of operable channels, one less than the total nwnber of channels, startup and/or power operations may proceed until performance of the next required channel functional test provided the inoperable channel is placed in the tripped condition within one hour. The channel was placed in the tripped condition for the calibration of the transmitter and the channel was declared inoperable on the basis of the "as found" data. Redundant channels were available throughout this occurrence.

LER 78-78/03L CORRECTIVE ACTION:

Difficulty in calibrating the transmitter in place resulted in the transmitter being removed and a bench calibration performed in the Performance Department shop. Data points were verified by an Instrument Supervisor and the transmitter was reinstalled. Comparison checks with other channels showed Channel 1 to be operating properly.

!This occurrence was caused by a zero shift in the transmitter caused by the shutdown
