SUMMARY
OF MEETING CONCERNING 9/12/85 UNUSUAL EVENT DURING LOSS OF LOAD TEST A meeting was held in Bethesda, Maryland on September 20, 1985 with representatives of the licensee to discuss the Unusual Event that occurred on September 12, 1985 during a loss of load test at about 505 power.
The meeting attendees are shown on Enclosure 1.
The backqround for and summary of the meeting are presented below.
Backqround On September 12, 1985, following the start of a loss of load test from about the 50% power level, Palo Verde Unit 1 experienced an unusual event which resulted in a number of unexpected occurrences.
These included a turbine trip, a lack of fast transfer of power from the generator to offsite power for non-essential
- loads, reactor trip, safety injection actuation (SIAS),
containment isolation and gas binding of the three charqing pumps.
Since the charging system is also used to provide water to the pressurizer auxiliary spray system, the auxiliary spray system was not available to perform its function, if it was required.
The following two things contributed to the gas binding of the charging pumps:
(1) a single failure in the volume control tank (VCT) level indicator, and (2) upon receipt of gn SIAS, shedding of power, by design, to the motor control center (MCC) for two of the valves (CH-501 and CH-536) which control water availability to the charginq pumps from the VCT and refueling water tank (RWT), respectively.
Because of the above, the staff questioned the design basis for the auxiliary spray system.
As a result, on September 16, 1985 the licensee agreed not to restart the plant until the design basis issue was evaluated by the staff.
Region V issued a confirmatory action letter on September 17, 1985 documenting the licensee's commitment.
By [[letter::05000528/LER-1985-048, Corrected LER 85-048-00:on 850819,ECCS Subsystem RQ & Bp Declared Inoperable Due to Noncompliance W/Surveillance Requirement 4.5.2.g.1.Temporary Change Initiated to Permit Testing of ECCS Throttle Valves|letter dated September 18, 1985]], the licensee proposed interim compensatory measures in support of continued operation of plant while a reevaluation of the auxiliary spray system was being performed to determine whether the system needed to be upgraded.
The September 20, 1985 meeting was held to discuss the September 12, 1985 event and the proposed interim compensatory measures for restart.

Meetin Summar (9/20/85 At the meeting, the licensee provided a more detailed discussion of the sequence of events that occurred on September 12, 1985.
The licensee indicated that the event had complications and the additional people that were available when the next shift arrived were helpful in recovering from the event.
During this presentation, discussions were held on the post-trip review process, vendor reviews of tests and hydrogen venting'of the charging pumps.
The design of the auxiliary pressurizer spray system (APSS) was also discussed.
The licensee stated that APSS was considered safety grade only in the context of complying with RSB 5-1 for achieving cold shutdown.
The staff indicated that APSS should be fully safety grade since APSS is ~elied upon for mitigating a postulated steam generator tube rupture (SGTR) accident.
The licensee stated that it is performing a reanalysis of the SGTR accident without reliance on APSS.
The compensatory measures for restart of Palo Verde Unit I were then discussed.
In addition to measures proposed by the licensee in its [[letter::05000528/LER-1985-048, Corrected LER 85-048-00:on 850819,ECCS Subsystem RQ & Bp Declared Inoperable Due to Noncompliance W/Surveillance Requirement 4.5.2.g.1.Temporary Change Initiated to Permit Testing of ECCS Throttle Valves|September 18, 1985 letter]], the licensee committed to other measures identified by the staff.
A summary of all the compensatory measures the licensee committed tn is shown below.
(I)
Monitor the reference leg of the VCT level indicator on a daily basis.
(2)
Revise appropriate procedures to require alignment of the RWT to the charging pump suction upon loss of offsite power, and institute procedural cautions on restart of the charging pumps.
(3)
Examine the post-trip review process to assure'hat the potential safety significance of off-normal events is adequately evaluated.
(4)
For the remaining power ascension test, assure that appropriate vendor input has been provided and determine if additional staffing on shift may be appropriate.
The staff stated that with the above compensatory measures it would recommend to Region V that Palo Verde Unit I be permitted to restart.
The staff informed the licensee that the long-term measures for upgrading the reliability of APSS would be the subject of a 50.54(f) letter.
