2. Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?
3. Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?

4. Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?
5. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?
6. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?
7. Does it appear that the area is free of potentially adverse seismic interactions Yes associated with housekeeping practices, storage of portable equipment, and WCAP-17678-NP November 2012 Revision 0

The space below each of the following questions may be used to record the results of judgments and findings.

Additional space is provided at the end ofithis checklist for documenting other comments.

1. Does anchorage of equipment in the area appear to be free of potentially Yes adverse seismic conditions (if visible without necessarily opening cabinets)?
2. Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?
3. Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

condition of supports is adequate ,and fill conditions of cable trays appear to be inside acceptable limits)?

Majority of overhead distribution',notedto be rod-hung but regularly supported with ductile connections. Moreover, the tributaryspans of overhead systems are low mass. Given adequate capacity due to regularsupport and ductile connections and low demand due to low mass, there is no adverse concern.

4. Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

Noted small clearance between fire piping and pipe stubs. No soft targets nearby and fire piping is well-supported. Therefore, no hazard.

5. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?

Noted wet threadedfire piping equipment in area but no equipment is sensitive to spray. Firepiping is regularlysupported so as to preclude falling.

Therefore no spray hazard.

6. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?
7. Does it appear that the area is free of potentially adverse seismic interactions No associated with housekeeping practices, storage of portable equipment, and WCAP-17678-NP November 2012 Revision 0

Location (Bldg, Elev, Room/Area): Area: CTRL, 1974, 3101 temporary installations (e.g., scaffolding, lead shielding)?

Noted sign posts located near safety related airtubing. Sign posts could fall and damage airlines. Ops corrected on the spot Also, sign posts were not set flat, but tilted.

8. Have you looked for and found no other seismic conditions that could Yes adversely affect the safety functions of the equipment in the area?

Overheadequipment is ruggedly supported; therefore no hazard. Chain-hung lights noted to have adequate clearance so as to preclude impact. No hazard.

4. Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

Noted two S-hook lights between diesel door and NGO2A bus with S-hooks not fully closed. Given that the hooks are not open enough for lateral excitation to cause hooks to disengage, the SWTjudged the lights to be no impact or fall hazard. Although no adverse seismic concern, SWT notified WCNOC to close for good practice.

5. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?
6. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?

7. Does it appear that the area is free of potentially adverse seismic interactions Yes associated with housekeeping practices, storage of portable equipment, and temporary installations (e.g., scaffolding, lead shielding)?

Noted equipment cabinetmoved away from its originallocation (adjacentto NGO02) to its currentlocation againstthe north wall as a result of IPEEE.

Currentlocation poses no hazard to equipment

8. Have you looked for and found no other seismic conditions that could Yes adversely affect the safety functions of the equipment in the area?

Noted missing panel bolt on NB0201. Panelserves no structuralpurpose; therefore no adverse seismic concern but notified WCNOC.

The SWT also noted NGO24A with doors open, but cabinet confirmed to be out of service. Therefore, no adverse condition.
