The space below each of the following questions may be used to record the results of judgments and findings.

Additional space is provided at the end of this checklist for documenting other comments.

1. Does anchorage of equipment in the area appear to be free of potentially Yes adverse seismic conditions (if visible without necessarily opening cabinets)?
2. Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?

Surface corrosion noted on flange bolts, but no indication of strength loss.

Therefore no hazard.

3. Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?

4. Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

Overheadlight shell noted againsta steel tube member. Lights confirmed to be on swivel bearing;therefore light impact with the steel tube member will not induce failure. No hazard.

5. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?
6. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?
7. Does it appear that the area is free of potentially adverse seismic interactions Yes associated with housekeeping practices, storage of portable equipment, and WCAP-17678-NP November 2012 Revision 0

B-61 Status: N U Area Walk-By Checklist (AWC)

Location (Bldg, Elev, Room/Area): Area : AUX, 2000, 1305 temporary installations (e.g., scaffolding, lead shielding)?

Scaffolding in NE corner of area noted to be well-supported with adequate clearances. Therefore no hazard.

8. Have you looked for and found no other seismic conditions that could Yes adversely affect the safety functions of the equipment in the area?

SWT observed various piping differently mounted from the floor, wall, and ceiling.

Given that the room and structure are reinforced and very stiff, the flexibility provided is more than adequatefor SSE demands. No hazard.

Noted lighting suspended that can impact piping. WCNOC Operations confirmed the piping to be plant air (NNS), and SWT informed all that the low mass of light cannot damage the pipe. No hazards.

The SWT noted the limiting case of piping crossing building gap with concern of overstress due to differential movement. 6" diameterpipe from containment to GS4VO021 with about 7' between support was noted to have adequate flexibility given pipe diameterand span. No hazard.

The SWT noted threaded fire piping in the area. OPS indicated that system is dry and manually activated from outside.

Therefore, no spray hazard.

Location (Bldg, Elev, Room/Area): Area : AUX, 2026, 1401 Instructions for Completing Checklist This checklist may be used to document the results of the Area Walk-By near one or more SWEL items.

Refer to Note 5 below regarding non-Q AHU on vibration mounts. No hazard.

2. Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?

Noted hoist well-supported with rig and chain in safe positions. No hazard.

5. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?

Noted non-safety related air handling unit SGL I 7B on rubber-vibration mounts, which historicallyperform poorly in seismic events. The concern is that the AHU anchoragecould fail, causing the unit to rupturepipes and create a flood hazard. Given that all equipment is well-protected and on raisedpads with adequatefloor drains, SWTjudged these to be of no seismic hazard.

6. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?

7. Does it appear that the area is free of potentially adverse seismic interactions Yes associated with housekeeping practices, storage of portable equipment, and temporary installations (e.g., scaffolding, lead shielding)?

SWT concluded that all scaffolding in areahas adequatesupports and bracing with adequate clearance. Therefore no hazard.

8. Have you looked for and found no other seismic conditions that could Yes adversely affect the safety functions of the equipment in the area?
