Additional space is provided at the end ofthis checklist for documenting other comments.

1. Does anchorage of equipment in the area appear to be free of potentially Yes adverse seismic conditions (if visible without necessarily opening cabinets)?
2. Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?
3. Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?

SWT noted four layer cable trays overhead supported rigidly with P1001 Unistrut member. The cable tray is laterally anchored to wall throughout.

There are no adverse seismic conditions.

4. Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment .in the area (e.g., ceiling tiles and lighting)?
5. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?
6. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?

3. Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?

Multi layered cable tray overhead throughout area is ruggedly supportedwith braced Unistrut frames, both trapeze and wall-anchored. Therefore cable tray is of no adverse seismic concern.

The SWT also observed vertical HSS cable tray supports welded to the floor and connected to the ceiling above. Connection at ceiling appears to be a jacket connection that only transfers lateralload. If the ceiling connection transfers vertical load, there is a concern the support's connections will fail in an SSE event due to differential floor displacement. Verification is requiredto determine whether either the ceiling connection transfers vertical load or if the support and its connections are designed for the verticalload transferdue to SSE.

Licensing Basis Evaluation: HSS section verified to only react lateralload at its top connection per Detail 3 on C-1408. Therefore, there is no adverse seismic condition.

4. Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

Observed a heavy, unanchoredequipment cabinet in NW comer of area, adjacent to phone conduit (NNS).:, Additionally, the SSE level excitation will not cause cabinet to move enough relative to conduit given its rigid support it sits on. Therefore, no hazards.
