INITIAL CONDITIONS Unit I startup in progress OPI1/A11102/00I (Controlling Procedure For Unit Startup) Enclosure 4.13 CRD Patch Verification in progress Tc = 342°F RCS pressure = 567 psig INITIATING CUE The Control Room SRO directs you to withdraw CRD Group Ito 50% per OP/I/A/l 105/019 (Control Rod Drive System) Enclosure 4.3 (Withdrawal Of Safety Rod Group I) beginning at Step 2.1.

The OSM has determined it is NOT desired to latch CRD Groups 2 thru 7 at this time.

STEP 1: Step2.l Perform the following: (R.M.)

Ensure RUN is ON.

Ensure SINGLE SELECT SWITCH to ALL.

SAT STANDARD: Determine control rod speed switch is selected to RUN by observing light indication on the Diamond.

Determine SINGLE SELECT SWITCH is selected to ALL on the Diamond. UNSAT Continue to Step 2.2 COMMENTS:

STEP 2: Step 2.2 and 2.2.1 Perform latch and P1 alignment of CRD Groups, as follows:

WHILE CRDs are moving, monitor the following indications:

  • CRD position Appropriate ranged SAT NIs
  • Startup Rate STANDARD: As CRDs are withdrawn monitor the above indications.

UNSAT Continue to Step 2.2.2 COMMENTS:

  • Group/Rod inserts to latch rods (CRD TRAVEL In light ON)
  • Group/Rod withdraws to test for latching (CRD TRAVEL Out light ON)
  • Group/Rod inserts (CRD TRAVEL In light ON)
  • Group In Limit light on Diamond will momentarily extinguish then illuminate.

SAT STEP 3: Step 2.2.2 Perform Latch and P1 alignment of Group 1, as follows: (R.M.)

A. Ensure GROUP SELECT SWITCH to 1.

B. Ensure only Group 1 CONTROL ON lights are ON. (P1 panel) {9} UNSAT C. Select LATCH AUTO.

D. WHEN Auto Latch is complete:

  • Verify LATCH AUTO is OFF.
  • Verify Group 1 0% lights are ON. (P1 Panel)
  • Verify Group 1 API/RPI indications agree. (P1 Panel)

NOTE: The Reactor is manually tripped prior to the Shutdown Bypass automatic RPS trip on Unit Startup. For this reason, only Group 1 is required to be latched during a Unit Startup when RCS pressure <2100 psig.

STANDARD: Rotate GROUP SELECT SWITCH to 1.

Determine that only Group 1 CONTROL ON lights are ON on the P1 panel.

Depress the LATCH AUTO pushbutton.

WHEN Auto Latch is complete:

  • Verify LATCH AUTO is OFF.
  • Verify Group 1 0% lights are ON. (P1 Panel)
  • Verify Group 1 API/RPI indications agree using the position switch on the P1 Panel.

Continue to Step 2.2.3 COMMENTS:

STEP 4: Step 2.2.3 IF RCS pressure < 2100 psig, perform one of the following:

  • Go To Step 2.3.

SAT

  • Perform the remainder of enclosure to latch Groups 2 thru 7.

STANDARD: Go To Step 2.3.

SAT STANDARD: Candidate performs EOP IMAs from memory (per OMP 1-18) and depresses the REACTOR TRIP pushbutton located on UB1.

Candidate continues to Step 2. UNSAT COMMENTS:

STEP 2: IMAs Step 2 Verify reactor power < 5% FP and decreasing.

SAT STANDARD: Candidate performs EOP IMAs from memory and verifies reactor power

<5% and decreasing and determines that reactor power is > 5% on all Nis.

STEP 3: Step 3 GO TO Rule 1 (ATWS/Unanticipated Nuclear Power Production).

NOTE: The IA HPI Pump is operating.

COMMENTS:

STEP 9: Rule I Step 6 CRITICAL STEP Start 1C HPI PUMP.

SAT STANDARD: Candidate locates the 1C HPI pump switch on 1UBI and rotates the switch to the CLOSE position and verifies white open light indicates off and the red closed lights are illuminated. 1C HPIP motor amps are UNSAT normal ( 60 amps).

Candidate continues to Rule 1 Step 7.

STEP 10: Rule 1 Step 7 Open the following: SAT 1HP-26 1HP-27 UNSAT STANDARD: Candidate locates I HP-26 on 1 UB1 and rotates the control switch to the open position for 1 HP-26. Verify the green closed light indicates off and the red open light is illuminated.

Candidate determines that 1 HP-27 is already open Candidate determines that 1 HP-26 did not operate as expected and refers to Rule I Step 7 RNO.

NOTE: I HP-26 is failed closed and will not open.

COMMENTS:

STEP ii: Rule 1 Step 7 RNO CRITICAL STEP

1. IF 1 HP-26 will NOT open, THEN open 1 HP-41 0.

SAT

2. IF at least two HPI pumps are operating, AND 1 HP-27 will NOT open, THEN perform the following:

A. Start the standby HPI pump.

B. Stop the 1C HPI PUMP. UNSAT C. Open IHP-409.

STANDARD: Candidate determines that I HP-26 will not open and then locates 1HP-410 on 1UB1 and rotates the control switch to the open position.

Verify the green closed light indicates off and the red open light is illuminated.

  • Verify flow is indicated in the B HPI crossover header.

Candidate verifies that two HPI pumps are operating and I HP-27 is open and continues to Rule 1 Step 8.

  • Flow verification NOT critical COMMENTS:

CRO-108 RO Page 9 of 11 STEP12 Rutel-Step8 Dispatch one operator without wearing Arc Flash PPE to open 600V CRD breakers on the following:

1X9-5C (U-i CRD Norm Fdr Bkr) (Ui Equipment Rm) SAT 2X1-5B (U-i CRD Alternate Fdr Bkr) (T-3/Dd-28)

STANDARD: Candidate dispatches one operator to open Unit 1 normal and alternate UNSAT CRD feeder breakers without wearing Arc Flash PPE.

Candidate continues to Rule 1 Step 9.

COMMENTS:

STEP 13: Rule 1 Step 9 Verify only two HPI pumps operating.

SAT STANDARD: Candidate verifies two HPI pumps are operating by verifying the red ON lights are illuminated for the 1A or lB HPI pumps and the IC HPI pump.

Verify HPI pump motor amps are normal for the operating HPI pumps. UNSAT NOTE: The standby HPI pump may start due to the opening of IHP-410 robbing RCP seal injection flow. If this happens, the student should continue to Rule I Step 9 RNO to secure the IA or lB HPI pump. Otherwise, this is the END OF TASK.

COMMENTS:

END OF TASK STEP 14: Rule 1 Step 9 RNO IF ll HPI pumps operating, THEN perform the following: SAT

_Secure 1AOR lB HPI PUMP.

Place standby HPI pump in AUTO.

UNSAT STANDARD: Candidate secures the 1A or lB HPI pump by placing its control switch to the OFF position. Verify the red ON lights are OFF and the white OFF light is illuminated. Verify motor amps on the pump secured read zero.

NOTE: This step is a Critical Step only if the standby HPI pump started when IHP-4l0 was opened in Step 11 of this JPM.

CRO-108 RO Page 10 of 11 CRITICAL STEP EXPLANATIONS STEP # Explanation 5 Step 5 is required to initiate a manual control rod insertion and start adding negative reactivity to the core.

7 Step 7 is required to align emergency boration from the BWST to increase boron concentration in the RCS which will lead to a reduction in reactor power by adding negative reactivity to the core.

9 Step 9 is required to start the IC HPI pump to ensure HPI flow in both HPI headers.

II Step ills required to align flow to the A HPI header since I HP-26 is failed in the closed position.

  • Adjust KHU-1 Generator speed with UNIT I SPEED CHANGER MOTOR until slow clockwise rotation of 13.8 KV SAT SYNCHROSCOPE is established.

2.6.2 WHEN 13.8 KV SYNCHROSCOPE pointer is 5° before reaching vertical, close ACB 1 KEOWEE I GENERATOR BKR.

UNSAT STANDARD: 2.6.1 Determine that KHU-1 is required to be synced to the grid and make the following adjustment:

  • UNIT 1 AUTO VOLTAGE ADJUSTER manipulated to match KEOWEE 1 LINE VOLTS with KEOWEE I OUTPUT VOLTS when 13.8 KV SYNCHROSCOPE pointer is vertical.
  • KHU-1 Generator speed adjusted with UNIT I SPEED CHANGER MOTOR until slow clockwise rotation of 13.8 KV SYNCHROSCOPE is established.

2.6.2 ACB 1 KEOWEE I GENERATOR BKR switch is taken to close when the 13.8 KV SYNCHROSCOPE pointer is 5° before vertical, and observes the red close light lit and the white open light off.

STEP 4: Step 4 SAT At 1A H2 ANALYZER (RP), perform the following:

  • Position SAMPLE VALVE SEL SW to PR-71 (TOP OF CONTAINMENT SAMPLE).

  • Position OFF / STANDBY / ANALYZE switch to ANALYZE.
  • Depress REMOTE SELECTOR pushbutton to ensure control is from the Remote Panel.

STANDARD: SAMPLE VALVE SEL. Sw. is positioned to appropriate sample valve:

  • PR-71 (Top of Containment Sample) for Train A Cue: Point to the Sample Valve Selector Switch and state switch is in the PR-71 position.
