1. Provide your rationale for expecting that the Cycle 15 non-destructive examination enhancements for freespan outside diameter stress corrosion cracking (ODSCC) are sufficient to reduce the incidence oflarge eddy current (EC) indications to nominal levels.

SNC Response The enhancements to the non-destructive examination (NDE) practices and guidelines consist of 3 measures:

a. Sleeve end narent tubg_ODSCC detection with 100% + Point examination:

The detection of ODSCC in the immediate vicinity of the upper (above the tubesheet) end ofinstalled sleeves with bobbin probes or Cecco probes was shown by the findings on R25C51 in SG B to be inefTective. Implementation of+ Point examination from the upper laser weld to approximately 1" above the sleeve end was an effective remedy. Testing in SG B during the 8/99 forced outage demonstrated that no additional sleeve end indications were present. Examination of all 3 SGs in the IRIS ISIidentified 3 indications in SG A,0 in SG B, and 3 in SG C, therefore, implementation of ETSS#96404 provided the required assurance that ODSCC in the parent tube section " shadowed" by the sleeve end will be adequately inspected.

Using a probability of detection (POD) approach, the prior inspections could be considered to have a POD equal to zero because of the use of a probe that would not react to such indications. Any improvement represented as a multiple of the prior performance would then be unbounded, i.e., infinite. Thus, to use the discussion of the improvement as some orders of magnitude relative to prior performance is not meaningful for this degradation. The additional training of analysts to be wary of this region of the tube and the high sensitivity of the + Point probe should efTectively eliminate the potential for missed indications of this type.

b. Freesnan ODSCC in tube sections free of appurtenances:

The leakage of R25C51 was attributed to freespan ODSCC ap ireximately 8" above a sleeve end; the 12.7V bobbin indication had grown fu. an unidentified (missed) 0.97V indication shown to have been present at the time of the IR14 ISI.

The behavior of this IR14 indication was consistent among the data channels and unencumbered by interference. By the guidelines associated with ETSS#96007, the qualified technique specific to bobbin detection of ODSCC, this indication should have been identified and reported as NQI. This would have required it to be examined and characterized by + Point probe. Given the concern over the growth from the unreported flaw, a special analyst orientation session was conducted to acquaint the analyst crew in the 8/99 forced outage with the characteristics of the indication. The indoctrination increased the emphasis on the review of small freespan bobbin indications to assure that signals such as the 0.97V 1R14 bobbin signal on R25C51 would be reported as NQI. Twenty-seven 1of8

confirmed freespan signals on 13 tubes were reported during the 8/99 forced i outage, none exceeding 2 volts; these additional tubes were plugged based on these results. The indications from R25C51 as well as the additional indications confirmed by + Point were added to the analyst training tapes for the IRI5 SSPD.

During IRIS after only 2 months additional operation,150 freespan indications in this category were confirmed out of 325 NQIs reported. Thus for Cycle 15 a doubling in the number of confirmed freespan indications was observed; since the growth study showed that the large majority of these tubes had measurable flaws at IR14, it is concluded that the training was effective to restore the POD for the technique to the nominal value (0.90@ 90%CL).

c. ODSCC Outside the TSP Edge; The SG leakage, ~20gpd, observed during the latter part of Cycle 15 was attributed l to ODSCC identified with a 27V bobbin signal just above the IH TSP in SG B on i

R43C32. Re iew of the IR14 bobbin dsta for this location revealed a 2.57V signal that had been identified but judged not to represent a flaw; this judgment was based on failure of the Lissajous trajectory to follow the "down first" trace typical of calibration flaws. This analysis " rule" is a part of the industry's fundamental analyst training, serving as a flaw-confirming characteristic for signals with phase j angles indicative of flaws, based on the behavior of standard flaws. As applied in this case, the rule served to eliminate from + Point examination a signal otherwise clearly characteristic of flaws with respect to phase angle. Technically, the IR14 l signal did not represent a " missed" indication, since it was consciously rejected on l the basis of the "down first" rule. l l

Upon realization of the identity of the IRI5 flaw location and the IR14 unreported signallocation, the IRIS analysis program was suspended after 400 tubes were analyzed. To remedy the discrepancy between the IRIS confirmed flaw and its l 1R14 precursor signal, a guideline change was implemented that required freespan  ;

signals with phase angles consistent with flaws in the detection channel (400/100 kHz mix) to be reponed as NQls, causing them to be examined by + Point probes to confirm and characterize the underlying tube condition. All shifts of analysts were indoctrinated with the history of the R42C32 signal development during a one-day analysis suspension for re-training; subsequently the analysis was re-started from the beginning so that all such signals present in the first 400 tubes and the balance of the 100% bobbin inspection would be identified. A total of 115 NQIs, from which 34 confirmed indications of this type were found in the inspection; 16 of the NQIs (3 NQCs) were found from the re-analysis of the first 400 tubes. Five of the 6 largest indications identified at 1R15 (including R43C32) were in this category; each had a precursor signal about I volt or larger, all of which would be reported as NQI under the revised instructions. The enhancement to the ETSS#96007 guidelines provides increased assurance that signals of a i magnitude historically associated with large growth cracks will be identified and l removed from service before they can present a challenge to tube integrity. Data from tubes representing the issues identified at Farley I were made available to the industry at large through EPRl's NDE Center; this facet of freespan ODSCC detection was incorporated into the training tapes for subsequent outages of the Farley I and 2 plants.

r Taken together, these measures satisfactorily address the causes of the tube leakage events

of Cycle 15 and provide increased assurance that the known mechanisms with a potential to produce freespan tube leakage are the subject ofinspections with properly implemented l

qualified techniques. It is seen that there are essentially three categories ofindications, free-span indications in the shadow of the sleeve end (1), free-span indications remote of signal altering features such as the TSP (2), and free-span indications near signal altering features that were easily detected, but omitted from further consideration based on the application of then existing guidelines (3). The use of the + Point probe to inspect the first type and the revised guidelines for the third type, coupled with the ease of detection, are sufficient to result in significant improvements in detection of these types of flaws. The enhancements discussed for the second type also show improved detection as shown by the inspection transient results discussed in question 3.

2. Show that the three pulled tubes you used to obtain a probability of detection correlation are not selectively favorable for detection relative to the general tube population with respect to noise levels.

l SNC Response Comparative measurements of the vertical component of bobbin data for each of the pulled tubes

as well as for the 20-tube sample population provided to the staff for their independent review L were reviewed. On each tube 5 corresponding locations were evaluated, to give a sense for the variability in the magnitude of signals that might interfere with the identification of flaws. The locations were chosen to bound the tube length on which virtually all the freespan ODSCC indications reported from bobbin probes analyses were detected. For each tube the calibration was established at the reference voltage in the 400/100 kHz mix for 20% flat-bottom holes, e.g.,

2.77 volts, determined for the pertinent ASME standard relative to the TSP ODSCC laboratory standard for voltage based ARC measurements. The data obtained using this measure of potentially interfering signals suggest that the tubes chosen had noise levels typical of the range of Farley 1 tubes as represented in the 20-tube sample provided to the NRC and were not selected to provide favorable results with respect to noise levels. In addition SNC did not consider noise to be a factor in tube selection when the data was supplied to the NRC staff.

It may be observed that the number of detected indications and confirmed indications both increased by more than a factor of 2 in total; examining the data for indications within 1.5" of the TSPs shows more striking increases,14-fold for NQIs and 7-fold for NQCs. This data illustrates both the increased readinesses of the analysts to make the NQI calls conservatively and the corresponding transient in confirmed indications that results. These results, together with the discussion of the issues presented in question #1, reflect the impact of the NDE enhancements implemented during Cycle 15.

This data represents the findings of the 100% bobbin inspection and the supplemental

+ Point inspection for the regions of the tube bundle designated as "freespan", i.e.,

unencumbered by sludge, sleeves, or support structures. The sludge zone for Farley I was designated as the length from the top of the tubesheet to 7.5" above the tubesheet prior to the IRIS inspection. This designation was based on the distribution of indications and the relative likelihood of detection by bobbin relative to + Point and represents a significant increase in the use of + Point probe over past outages. + Point vs. Bobbin detection behavior for above the top of the tubesheet ODSCC was 5 of 8

performed to assess the performance of the bobbin probe for detection of free span degradation above the sludge deposit zone. In the 1R13 inspection the nominal industry

+ Point inspection length of 3" above the tubesheet was used. The IR14 inspection covered up to 5" (nominal) above the tubesheet; in fact, inspection coverage reached approximately 7.5" ATS. Tubes with a + Point indication above 7.5" also had a bobbin indication above 7.5", i.e.,100% matching was found, though not always mapping perfectly; this was regarded as sufficient corroboration because of the bobbin probe's ability to eliminate some interfering signals in the mix channel. It was determined that for 1R15 the required inspection length for the 100% + Point hot leg inspection program should be increased to cover the length up to at least 7.5" (nominal) above the tube sheet (ATS); in fact, inspection coverage reached approximately 9 inches ATS NRC Ouestion

4. Provide revised projections for the probability of flaws occurring at 144 efTective full power days (EFPDs) and 409 EFPDs in the current cycle as a function of the

- stress intensity magnification factor (Mp) of the flaws. Again, as most recently provided, the probability of occurrence should be for the worst flaw in the generator (i.e., the statistical distribution for the largest Mp in each of the 250,000 Monte Carlo cases). However, in order to support the stafi's intended use for estimating the probability of tube failure under core damage accident conditions, the formula used for calculation of Mp must be the same as was used by Argonne National Laboratory for their correlations of data on flawed tube failure due to creep under high temperature conditions Specifically, Mp needs to be calculated as described in NUREG-1570 in section 4.3.2.3 " Improved Correlation for Mp," using formula 18c on page 4-22.

Thus, the calculations did use the Argonne model for ligament tearing and for throughwall burst. The simple difference is the distribution of the maxima of the burst magnification factors was supplied when the staff wanted the distribution of the maxima of the onset-of-leak magnification factors. For the severe accident evaluations, failure is being defined as ligament tearing regardless of whether or not the resulting throughwall crack will run. Of course, once the ligament has torn the burst of the tube is governed by the magnification factor previously provided. A large release leak rate would not be expected from a crack with a short throughwall length. The current simulation results are the distribution of the maxima of m, only, which are inversely proportional to the distribution of the minima of the onset-of-leak pressures.

The calculation of the maximum m, value for any given crack profile proceeds similar to the method for determining the minimum burst pressure for that crack.

The magnification factor for each discrete subsection and combination of contiguous subsections is calculated and the maximum value is taken as applicable to that crack. This means however that the maximum m pvalues are. almost always l 7 of 8 1

based on the value found for the deepest ponion of the crack being simulated, even when that length is very short. For example, a tube with a throughwall crack on the order of 0.05" long has an infinite m p. To resolve this concern it was agreed that the values ofmp will be calculated only for sub-crack lengths greater than 0.25" To include consideration of the tubes in the " hot-zone" of the SG, the simulated values ofm pfor each simulation of the SG are ordered from smallest to largest. The value ofm pto be retained as the maximum for that simulation is the largest value such that s sample pseudo-random uniform distribution number is less than or equal to 0.53. For example, suppose 30 values are simulated and ordered. Funher suppose that three draws from a uniform distribution are needed to obtain a number s 0.53 (the first two draws being > 0.53). The applicable maximum value ofmp expected in the hot-zone is that corresponding to the 28* ordered value with the 29*

and 30* occurring in the cold-zone of the SG tubes.

Finally, in order to expedite the calculation process, the minimum acceptable total number of simulations was reduced to 30,000. The rationale for this is that the number of simulations needed to confidently quantify the occurrence of an event is equal to 30 times the inverse of the probability of the event being sought. Hence, 30,000 simulations should easily quantify the occurrence of events which individually have a probability of occurrence on the order of 0.001. If suflicient time existed for the analysis, e.g., runs staned at the end of the work day, the number of simulations may have been increased to some convenient number greater than 30,000. Enclosure 1 contains the information requested.
