Statement of Violation:
Technical Specification 6.8.1 requires, in part, that written

procedures shall be established, implemented, and maintained,
'
including the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
Item 1.e of j
Appendix A includes administrative procedures for procedure review j
and approval.
Procedure OPGP03-ZA-0002, " Plant Procedures",

Revision 22, dated July 1, 1992, required-procedure reviewers to l
" include all active Field Changes against the old revision of the
l procedure" in the revision process.

Contrary to the above, Surveillance Test Procedure 1 PSP 03-SP-0009C,.
.
"SSPS Actuation Train C Slave Relay Test," was revised on September j
15, 1992, but did not include the change implemented by Field Change i
92-0424.
This is a Severity Level IV violation (Supplement I) (498/9320-02)

Reason for the Violation:

i Survej ' lance Test Procedure 1 PSP 03-SP-0009C, "SSPS Actuation Train C
Slave Relay Test," Revision 2, was issued without Field Change (FC)
92-0424 being incorporated because the procedural requirement to
incorporate or delete active FCs was not followed.
FC 92-0424 was originally issued because a procedural step had been misread, and Auxiliary Feedwater Pump 13 was inadvertently started during a previous performance of the procedure.
FC 92-0424 changed the wording in the procedure from " verify that AFW Pump 13 did NOT start" to " verify that AFW pump 13 has NOT started."
The inadvertent start of AFW Pump 13 was caused when the step was misinterpreted to mean that "an attempt to start the pump should be made, but the pump should not start", when the step was only meant to be an observation that the pump was not running.
The draft of 1 PSP 03-SP-0009C, Revision 2, was distributed for review and comment on March 4, 1992.
FC 92-0424 was approved on September
.9, 1992.
Because FC-0424 was issued after the procedure revision was distributed for review and comment, the Procedure Approval Form did not list FC-0424 as being applicable.

The Procedure Approval Form was updated to include FC 92-0424 on September 14, 1992, because it was believed that FC 92-0424 was already incorporated in the revision.
1 PSP 03-SP-0009C, Revision 2, was subsequently approved on-September 15, 1992 without FC 92-0424 incorporated.
Because FC-0424 was only a clarification of the wording in 1 PSP 03-SP-0009C, omission of this FC had no safety significance.
Between September 15, 1992, when 1 PSP 03-SP-0009C was approved, and 1993, when clarified wording was returned to the procedure, June 4, the procedure was performed three times without an incident similar to that which previously occurred.
IV.
Corrective Actions:
The following corrective actions have been taken:
1.
Field Change 93-0980, dated June 4, 1993, has been incorporated into procedure 1 PSP 03-SP-0009C to change the wording of the step back to " verify that AFW pump 13 has NOT started."
2.
A memorandum from the Vice President, Nuclear Engineering was distributed to Nuclear Engineering personnel stating the lessons learned from this event and stating management expectations regarding future procedure revisions.
V.
Additional Information:
Procedure OPGP03-ZA-0002, " Plant Procedures", requires review of active FCs both by the preparer of procedure revisions and the Procedure Control Group (PCG) to ensure that FCs are incorporated into or deleted by the next revision of the procedure.
The PCG review is administrative in nature and was implemented as corrective action for previous occurrences of FCsid:t being incorporated.
The additional review by PCG was not effeg
