Statement of violation: Technical Specification 3.7.7.a., requires that, with one control room makeup'and cleanup filtration system inoperable in Modes 5 and 6, the inoperable system be in operable status within 7 days or initiate and maintain operation of the remaining operable control room makeup and cleanup filtration systems in the recirculation and makeup air filtration mode.

Contrary to the above, on March 10, 1993, with Unit 2 in Mode 6 operation, plant operators failed to place Trains A and C control room makeup and cleanup systems in the recirculation and makeup air filtration mode with Train B out of service greater than 7 days.

This is a Severity Level IV violation. (Supplement I) (499/9311-01) II.

Houston Lichtina & Power Position: IIL&P concurs that the violation occurred.

III.

Reason for Violation: The Station was relying on an uncontrolled printout of the operability Tracking Log (OTL) index produced by a computer program that had not been validated by procedure. An error in the program caused incomplete information to be listed in the index. Thus, the Control Room staff did not realize that the seven-day Limiting Condition for Operation would expire during their shift.

Corrective Actions: l At 1312 hours on March 10, 1993, Trains A and C of the CRE HVAC system were placed in the filtered recirculation mode of operation to comply with TAchnical Specifications 3.7.7a and 3.3.3.7b.

The following actions have been or are being taken to prevent recurrence: 1. The use of the computer-generated operability Tracking Ing (OTL) index has been suspended pending correction of the computer program.

A manually-updated index is being maintained within the control area of the control room.

2. A review was performed to determine if other operations-related computer programs which may cause a similar . problem are in use.

Three other programs are in use; two-of the programs have been validated per procedure.

'the third program, Equipment Clearance Orders, will be , validated by June 25, 1993.

3.

Licensed Operator Requalification Training will reinforce the different modos of operation for the CRE HVAC system , including the Technical Specification requirements for l cach mode. This action will be completed by July 9, 1993.

Statement of Violation: Eg_i_lu re to Follow Procedures for Restoration of Electrical , _ Inverter Technical Specification 6.8.1.a requires that written proceduree ' shall be established, implemented, and maintained covering those activities recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Regulatory Guide 1.33, Appendix A, Item 3.s requiros procedures for startup of electrical systems.

Plant procedure 1 POP 02-NA-0001, Revision 6, "Non Class 1E 120 VAC , Distribution Power Supplies," Step 7.1.4, requires that the associated capacitor bank be procharged prior to placing Inverter l ' N1 in service.

j Contrary to the above, on March 29, 1993, a reactor plant operator placed Inverter N1 in service without first precharging the associated' capacitor bank.

This la a severity Level IV violation.

- _eason for Violation: R III.

An adequate pre-job briefing was not performed because of the high level of administrative activity in the Control Room, i.e., ' verbal or written instructions for powering up Inverter N1 for

the load test about to be performed were not provided.

  • I A partial release of the Equipment Clearance Order (ECO) on Inverter N1 was authorized to remove tags from two breakers and reposition them for performance of the load test.

Use of the system operating procedure, 1 POP 02-NA-0001, was not required because the tags were being removed under control of the ECO procedure, OPGP03-ZO-0039, " Operations Configuration Management."

(Restoring partially released equipment is not routinely performed using the system operating procedure because the ECO provides a restoration sequence.)

However, this ECO did not i specify the restoration sequence or that the inverter capacitor bank required precharging.

Also, the work instructions for the load test did not contain the requirement to precharge the capacitor bank.

A contributing cause to this violation is that_only some inverters require manual action to procharge the capacitor bank.

Although operator training includes operating the inverters that require manual action, inverter operation is an infrequently t performed task, and the only reminder to procharge the capacitor

bank is in the system operating procedure.

IV.

porrective Actions: ' The following corrective actions have been taken to prevent recurrences 1. An operations Work Control Group, formed to reduce the administrative burden on the Senior Reactor Operators, has ' been demonstrated to be effective.

2.

ECO forms have been modified to ensure that adequate work instructions are being provided when equipment and systems are partially released and are not being returned to their normal operating lineup.

A block has been added to the ECO form to indicate whether a system operating procedure is to be used ) when tags are being removed and components are being realigned.

If the ECO does not provide adequate instruction

for realigning the component, the system operating procedure ' is used.

3.

Labels have been placed on those inverters which require

manual action to precharge the capacitor bank.

Statement of violation Technical Specification 3.1.2.1.

states that, as a miniums, one boron injection flow path shall be operable and capable of ' being powered from an operable emergency power source in Modee 4, 5, and 6. With none of the flow paths operable or capable the of being powered from an operable emergency power source, Technical Specification requires suspension of all operations involving core alterations or positive reactivity changes.

Technical Specification 3.1.2.3. states that one charging pump in the boron injection flow path required by Technical Specification 3.1.2.1. shall be operable and capable of being powered from an operable emergency power source in Modes 4, 5, and 6. With no charging pump operable, or capable of being powered from an emergency power source, the Technical Specification requires suspension of all operations involving core alterations or positive reactivity changes.

Contrary to the above, on April 1, 1993, with Unit 1 in Mode 5 operation, no operable boron injection flow path existed during control rod and digital rod position indication testing, which constitutes operations involving positive reactivity' changes.

This is a Severity Levc3 IV violation (Supplement I) (498/9311-04).

II.

Houston Lightina & Power Position: HL&P concurs that the violation occurred in that centrifugal charging pump (CCP) 1A was administrative 1y inoperable as described in IER 93-013, Unit 1.

It should be noted that a borated flow path through CCP 1A was functionally available; the ability to charge borated water into the Reactor Coolant System had been demonstrated.

Regarding the availability of minimum coolant flow to protect continued pump operation, CCP 1A would have had adequate cooling flow even if the discharge path to the reactor coolant system (RCS) cold leg and the recirculation path through FCV-201 had both boon isolated. The flow from the CCP to the i reactor coolant pump (RCP) neals is nominally adjusted to 32 j gpm while the required minimum cooling flow for the CCP is 30 gpm. Therefore, except for the extremely unlikely combination ' of a spurious safety injection signal and RCP seal isolation, CCP 1A would have been available to provide horated water to the RCS if neccosary.

III.

Reason for Violation: The reason for. the violation was the use of a less than effective system for tracking inoperable Technical Specification equipment and actions during an outage. The.

i tracking system allowed control room staff to forget - the administrative status of CCP 1A.

A contributing reason was that control room supervision reasoned that CCP 1A was completely capable of performing its safety function without interruption as described in Section II above.

Additionally, an inadequate review of the operability Tracking Log (OTL) was performed prior to shifting from CCP 1B to CCP 1A and again lator during the Control Room staf f shift change.

, IV.

ggrrective_ Actions: The required post-maintenance tests were performed on CCP 1A and it was declared operable at 0318 hours on April 2, 1993.

The following actions are being taken to prevent recurrence: 1.

Removable labels have been mado available to place on the main control board to remind personnel of important Technical Specification equipment status. Additionally, an information board highlighting key Technical Specification action times and dates was placed in the control room.

2.

This event will be included in Licensed operator- ' Requalification.

This action will be completed by August 31, 1993.

F. Groth (ML&P) to NRC Document Control Desk, dated June 18, 1993 (ST-HL-AE-4468) In the referenced letter, Houston Lighting & Power (HL&P) stated that a review was performed to determine if othar operations-related computer programs which may cause a similar problem were in use.

As stated, the review determined that three other programs were in use and that two of the programs had been i validated.

A commitment was made to validate the third program by June 25, 1993.

HL&P notified the Resident Inspector by telephono on June 24, 1993, that the response was being supplemented and that the committed date would not be mot.

The third program is a specific application of a commercially available combination word processor and relational databasc.

It reduces the administrative burden on the control room staff by , generating a draft form for use as an Equipment Clearance-Order l ' (ECO).

It also produces a list of equipment tag numbers that are related to that specific ECO, which are manually verified against controlled documents prior to use. The program has previously been , reviewed by Plant Operations Department personnel line-item by j line-item.

HL&P's long term plan is to upgrado the ECO computer program to allow its use on a stand-alonc basin to further relieve administrative burden on the control room staff.

Prior to its-use in this manner, a full validation and verification w111 be completed by midyear 1994.
