Technical Specification Table 4.3-2, " Engineered Safety Featurce Actuation - System Instrumentation Surveillance Requirements," states that the High-1 containment pressure channels have to be channel checked each shift when in operational Moden 1 through 4.

Contrary to the above, during the period February 4-6, 1993, the Unit 1 chift channel check requirement of Table 4.3-2 for containment pressure (High-1) had not been accompliched ao required.

Thoso two examples constitute one Severity Level IV violation.

Correctivo Actiong: A.

General Management is continuing its efforts to stress the importance of attention to detail.

Recent examples of thin effort include: 1.

Identificetion of a station lluman Performance specific goal of helping line Coordinator with a departments improvo huma?1 performance.

2.

Implementation of the Plant Manager's Forum to address issues such as personal performance, including attention to dotail and self-verification.

3.

Initiatives, such as setting aside one day a week where station meetings are minimized, aimed at getting management and supervision into the field and in contact with station personnel.

This allows the opportunity for management to personally reinforce performanco expectations and affords station personnel the opportunity to provide feedback directly to their supervisor and/or manager.

l 4.

Site vide group meetings to discuss human performance incuoc and to solicit suggestions from station I personnel on how to improve human performance.

D.

Inoperabic Overtemperature/ Delta Temperature Channels 1.

Unit 1, RC-0430 Temperaturo loop was recalibrated on January 15, 1993, prior to entry into Mode 2.

Results were satisfactory.

2.

Unit 1, RC-0455 and RC-0456 Pressurizer Pressure loops were recalibrated on January 14, 1993.

Results were antisfactory.

3.

Affected calibration procedures have been corrected.

4.

The human performance errors that caused the procedure i error, incorrect calibrations, and nubsequent repeat work activities have been reviewed with Maintenance Department Proceduro writers.

Containment Prensure High-1 Channel Chocks 1.

An archival computer report was generated to verify that the containment pressure channels had been within the required tolerancos while in Mode 4.

2.

The operator log in the procedure was corrected to reficct tho Technical Specifications requirement for Containment Prosauro High-1 channel checks.

3.

The Technical Spocification-related operator logs were reviewed for accuracy and completeness.

No other Technical Spocification discrepancies were found.

4.

HL&P has developed a surveillance procedure enhancement program to ensure that surveillance procedures accurately reflect the design basis and adequately , perform the Technical Specification intended function.

Date of Full compliance: A.

Inoporable overtemperaturo/ Delta Temperature Channels HL&P has been in full compliance since April 15, 1993, when the remaining calibration procedures utilizing the incorrect curve were revised.

B.

Containment Prossure High-1 Channel Checks HL&P has been in full compliance since February 6, 1993, , whon the Operator Log was changed to incorporate the required channel check.

statement of violation: Technical Specification 6.8.1 statos, in part, that written procedures chall be established, implomonted, and maintained, including the applicable proceduros recommended in Appendix A of Regulate-J. Ado 1.33, Revision 2, February 1978.

Regulatory Guide 1.33, Appendix A, Paragraph 9, includes procedures for performing maintenance.

Paragraph 9.a of Regulatory Guide 1.33, Appendix A, recommends, in part, that maintenance that dan affect the performance of safety related equipment should be properly preplanned and performed in accordance with documented instructions appropriate to the circumstances.

Contrary to the above, on January 28, 1993, an unplanned Engineered Safety Foaturec actuation occurred because the preventive maintenance work instructions were not appropriate to the circumstancoc and were not proporly performed.

This is a Soverity Level IV violation.

(Supplomont I) (499/9304-02) II.

Houston Lichtino & Power Position: HL&P concure that the citod violation occurred.

III.

Roason for violation: Most of the levol transmitters at STP are of the " wet leg" type, requiring the une of a Wet Calibrator during calibration activj ties.

However, as in thic cituation, there are certain applications where " dry leg" transmitters are utilized.

Each requires a different type of Measuring and Testing Equipment (M&TE) be used during calibration activition.

Failurc to use the proper M&TE can result in falso indication and signals when returning the transmitter to service.

Tho Provontative Maintenance (PM) instruction for this particular activity did not identify the transmitter was a " dry lega nor opecifically identify the correct type of equipment to be used during the calibration.

The I&C Technicians did not recognize the application of the transmitter and used the wrong M&TE.

Use of a Wot Calibrator during calibration and subsequent venting of the transmitter in preparation for returning the device to service, croated a faloc lovel cignal.

This actuated a lovel differential switch connected in parallol with tho transmitter.

Actuation of this switch rosulted in the start of the ESF equipment.

Raason for violation (continuedit Two contributing factors were also identified.

The first was'that the PM instructions did not specifically identify the parallel process-connected ESF actuation equipment (level differential switch) and second, the level differontial switch was not labeled , i as an ESP actuation device.

IV.

Corrective Ac_tions: 1. The identified PM instructions have been revised to add a caution statement that specifies the correct type of test equipment.

2.

ESF actuation labels have been placed on local instrument cabinets for ECW trains in both Units.

Additionally, ESF actuation labels have been affixed to the ESF actuation instrumonts inside the cabinets.

3.

ECW PM instructions have been revised to add steps in the precautions notifying the performer of the parallel process-connected ESF actuation instruments.

4.

This avont has boon discussed with I&C personnel.

This discussion covered aspects of this event with regards to attention to the type of work performed and the equipment being worked on.

Additionally, job ownership, work habits, and attention to detail were discussed.

S. HL&P has performed an ovaluation to determine if the need for . additional training on the use of Measuring and Testing i Equipment is required.

The evaluation concluded that the t current training on the use of M&TE is adequate and additional instruction is not necessary.

6.

HL&P has completed a review of similar PM activities to ensure that proper cautions exist so that the correct type of calibration is usod for the application and is currently in the process of adding the necessary precautions.

PMs identified as needing additional clarification will be revised by May 28, 1993.

V.

Date of Full Compliance: IIL&P will be in full compliance by May 28, 1993, upon completion of PM upgrades.

statement of Violation: Technical Specification Soction 6.2.2.b states, "at least one licensed operator shall be in the control room when fuel is in the reactor.

In addition, while the unit is in Mode 1, 2, 3, or 4, at least one licensed senior operator shall be in the control room."

Contrary to the above, on February 14, 1993, the Unit 2 control room was not staffod by a senior reactor operator for 41 seconds while the unit was in Mode 4 operation.

This is a Severity Level IV violation.

(Supplement I) (499/9304-03) II.

Houcton Linhtinn & Power Position: HL&P concura that the cited violation occurred.

III.

Rean_on for Violation: The reason for this violation was inappropriate actions on the part of the Unit Supervisor.

The Unit Supervisor was cognizant of his responsibi~.ity to maintain control room command function in the absence of the Shift Supervisor.

Ilowever, his preoccupation with the surveillanco in progress caused his loss of focus on his primary responsibility, resulting in a violation of Technical Specifications.

, IV.

Correctivo Actions: 1.

A Human Performanco Review Board was conducted with the Unit Supervisor and representatives from Operations, Training, Correctivo Action Group, and Management.

As a result, the Plant Manager will discuss the command and control function requirements with Licenced Senior Operators filling the positions of Shift Supervisor and Unit Supervisor.

These discussions will completed by June 11, 1993.

2.

The Licensed operator Licenso Maintenance procedure will be revised to add a required discussion between Licensed operators and Plant operations Manager concerning command and ' control functions.

Theco diccucsions will be held prior to License Activation Cortification. The procedure revis1on will be completed by August 9, 1993.
