Inservice Inspection a.
General This is a maintenance and fuel conservation outage. UE contracted i
with Westinghouse LW) to perform the steam generator eddy current inspection and with Nuclear Energy Services (NES) for the balance of the ISI except for selected visual examinations which were performed by UE QA. The ISI was conducted in accordance with the ASME Boiler and Pressure Vessel Code,Section XI, 1980 Edition, Winter 1981 Addenda.
Steam generators B and C were eddy current inspected and the following 5 tubes were administratively plugged:

108-Although the tube indications did not exceed Technical Specification limits, it was decided by UE to plug them, since the growth rates of the indications could not be projected, due to the lack of historic data.

3.
Steam Generator Sludge Lancing a.
General Tubesheet cleaning is performed to remove sludge deposits which have-accumulated on the secondary side of the tubesheet during plant operation.
It is accomplished by means of injecting high pressure water (1500-2000 PSI) between the columns of tubes on top of the tubesheet. Simultaneously, water at low pressure (approximately 150 PSI) is injected through one handhole directed around the periphery of the steam generator and removed from the opposite handhole along with the high pressure flow. The action of both-streams of water provides a constant high volume washing of the tubesheet to remove deposits dislodged by the high pressure flow.
The water is removed from the generator, filtered and returned to a j
system holding tank. The quantity (volume) of sludge removed from the steam generator is determined by the filter weighing method.
Sludge lancing was perfonned by Westinghouse on four steam
generators. The total wet weight of sludge removed was 45 pounds, b.

Training and Qualification Effectiveness

An evaluation of training and qualification effectiveness was made during

the inspection. The evaluation included discussions with ISI personnel and observations of ISI personnel performance during work activities.

On occasions ISI personnel have been observed during discontinuity evaluation. Their performance during such evaluation demonstrated ability to identify and evaluate discontinuities, and is indicative of effective training. The ISI personnel appear to be knowledgeable of industry practices.
No violations or deviations were identified.

Exit Interview The inspector met with the site representatives (denoted in persons contacted paragraph) at the conclusion of the inspection. The inspector

summarized the scope and findings of the inspection noted in this
