Inspection on May 29 - June 1, 1979 (Report No. 50-458/79-01)

Areas Inspected: Routine, announced inspection of safety related concrete specifications; licensee action on previously identified inspection findings; preparations for resumption of construction; and off-site visit to local PDR.

The inspection involved twenty-four inspector hours by one NRC inspector.

Results: Of the four areas inspected, no items of noncompliance or deviations were identified.

Licensee Action on Previous Inspection Findings (Closed) Unrerolved Item (50-458/78-01: 50-459/79-01):

Plant Site Excavation and Backfill haintenance. The IE inspector reviewed Stone & Webster (S&W)

Specification BBS-210.100 (Rev. 1), March 29, 1978, " Site Development Work,"

in regard to removal of excavation overfill material.

Specification BBS-210.

100, page 1-27 requires that previously placed fil! material be stockpiled separately and tested for gradation. Gradation will be performed in accord-ance with ASTM Cl36-71 for each 5000 cubic yards removed.

S&W has also issued a contract for plant excavation to foundation grade and for the elimination of erosion on the slopes of the main hole excavation.

Purchase Requirements No. 38 and 46 of the contract require the contractor to remove fill material to proper grade level, to transport material to pre-determined spil) areas and to permit gradation tests by S&W FQC to determine if gradation requirements are being met.

In addition, the contract calls for removal of approximately 65,000 cubic yards of silty material to eliminate tracks of erosion and to prcvide a protective covering on the slopes to prevent further erosion.

The protective covering will be applied only to slopes that will not be covered with fill material.

This item is considered resolved.

(Closed) Unresolved Item (50-458/78-05):

Dewatering System Pump Engine Supports. The dewatering system pump engines have been placed on concrete blocks. Erosion by rainwater under the engine supports has been stopped.

This item is considered closed.

Site Tour and Status of Construction The IE inspector made a general tour.of the site accompanied by a li 1see representative. The site has been activated since Febr"ary 1979 in order to make preparations for resumption of construction vork. The batch plant.

is being prepared for operatton by National Mobile. Delta Testing and Inspection, Incorporated, has been contracted to conduct concrete materials qualification tests as well as to perform the concrete design mix testing.

Rour.ine field testing of concrete and concrete materials will be done by the constructor, Stone & Webster.

Excavation down to grade level has been contracted to Morrison-Knudson Construction Company. They will also repair the e:ccavation slopes and remove contaminated fill materials. Excavation was scheduled to begin on June 4, 1979.

The dewatering system (Wellpoint Dewatering Corporation) is beit.g run twenty-four hours per day in order to lower the water table to at least ten feet below construction grade level.

The licensee representative estimated that Category I concrete placement would begin about July 15, 1979.

Construction will be on Unit 1 only.

Unit 2 has been delayed for an indefinite period of time.

No itec3 of noncompliance or deviations were identified.

Construction Deficiency Report - Breakdown in Quality Assurance Program for Reinforcing Steel The licensee representative reported to the IE inspector that all noncon-forming rebar has been removed from the construction site to a designated area (DE-4) which is outside the controlled area fence. This area is on company property but approximately one mile from the Police Tury Road

gate which leads into the construction area.

The IE inspector reviewed N&Ds (Nonconformance and Dispo:. tion Reports)

relating to the rebar deficiency and discussed the matter with licensee representative. The following were determined:

Part of the nonconforming Category I rebar has been reclassified Category II.

An implementing procedure (FRI-S10.61-02) to control non-Category I c.

rebar has been approved but has not been implementd.

Any additional rebar received on site will be Category I rebar.

The licensee representas_ve stated that the procedure for controlling non-Category I rebar would be implemented and all non-Category I rebar would be appropriately marked (with paint) and controlled prior to start of any rebar usage in safety-related structures. Non-Ca tego ry I rebar will not be brought inside the controlled area fence until properly identified.

This item is considered unresolved and will be examined during the next NRC inspection.

Organization On April 9,1979, GSU reported a reorganization cf the Engineering and Construction Department (Figure (1)). Responsibility for the River Bend Project is as follows:

Mr. A. C. Baker, Director Projest Adminiatration Mr. W. J. Reed, Supervisor Project Licensing The Quality Assurance organization, under Mr. Tom Crouse, will be located at River Bend. The QA functional organization is shown in Figure (2).

A total QA force of eleven professionals has been authorized for the River Bend Station. The Supervisor, Operations QA will be announced at a later date.

6.

Local Public Document Room (LPDR)

The IE inspector visited the LPDR in St. Fransville to examine the facilities and to determine if material relating to River Bend is being received. The LPDR is located in the public librarv. The LPDR material is stored on open 443 310

shelves under control of the Librarian.

Due to the press of work (only one person in the library), LPDR information is not filed or sorted except on occasion by a volunteer. Material for River Send is received on a regular basis.

The Librarian stated that there are virtually no requests for use of the material.

7.

Safety-Related Structural Concrete - Review of Specifications The IE inspettor reviewed three Stone & Webster specifications that relate to concrete and concrete testing.

Title 210.350 Mixing and Delivering Concrete 210.360 Concrete Testing Services 210.370 Placing Concrete and Reinforcing Steel As a result of the review and discussions with licensee representatives, three questionable areas were identified that relate to concrete production and concrete materials. The licensee representative stated that these areas would be reviewed with the contractor. These areas are as follows-a.

Review of controls associated with any water withheld from a concrete batch for retempering purposes.

b.

Review of~ specification requirements for approval of design mixes 100 days prior to intended use.

c.

Review of quality controls for river barge delivery of cement from Illinois to Baton Rouge silos and subsequent trucking to the site.

The IE inspector stated that these areas would be examined during the next NRC inspection which would be conducted prior to placement of safety-related concrete.

No items of noncompliance cr deviations were identified.

8.

Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance, or deviations.

An unresolved item disclosed during the inspection is dis-cussed in the following paragraph:

Exit Interview The IE inspector met with licensee representatives (denoted in paragraph 1) at the conclusion of the inspection on June 1, 1979.

The IE inspector summarized the scope and findings of the inspection.

The unresolved item concerning the reinforcing bar (paragraph 4) was ack-nowledaed by the licensee representative.
