Inspection Summary:

Inspection on June 25-28, 1979 (Report No. 50-458/79-02)

Areas Ir.spected:

Routine, unannounced inspection of preparations for placement of safety-related concrete; licensee action on previously identified inspection findings; and quality assurance program review. The inspection involved fifty-six inspector hours by two NRC inspectors.

Results: Of the three areas inspected, no items of noncompliance or deviations were identified.

Licensee Action on Previously Identified Inspection Findings (0 pen) Unresolved Item:

Construction Deficiency Report - Breakdown in Quality Assurance Program for Reinforcing Steel (50-458/79-01).

(Control of Non-Safety-Related Reinforcing Steel.) The inspector examined the progress of the constructor in marking and controlling non-Category I rebar.

The rebar was being marked with paint according to the prescribed proce-dures. FQC personnel were observed monitoring the activity. A controlled area (gate and lock) has been established.

Due to the long term nature of this item it is not considered resolved at this time.

Site Tour The IE inspector conducted a tour of the River Bend site to observe con-struction activities.

The excavation of over-fill material in the Unit 1 area was in progress. A temporary pad for assembling of rebar mats for the Containment Building base mat had been constructed in the area adjacent to the excavation.

, Stone and Webster Quality Assurance Org.inization The IE inspector reviewed the onsite QA Organization and plans for Stone and Webster (S&W), Field Quality Control (FQC) coverage.

The functional FQC organization is shown in Figure (1). The organization shows two teams (A and B) which will conform to the general site working schedule of

" Rolling 40's*."

Not all positions are filled, however there are adequate numbers of FQC personnel on board for the present work activity.

No items of noncompliance or deviations were identified.

Procedure Review The IE inspector commenced review of procedures, and other safety-related documents as listed below.

S&W Quality Standard QS-5.1, Revision C (CN#1 and 2), August 9, 1978, describes the documents to be used by Stone and Webster in the conduct of construction activities at the River Bend Project.

Concrete Mix Design Review The IE inspector commenced review of the concrete mix design qualification as required by Specification 210.350, " Mixing and Delivering Concrete".

Engineering and Design Coordination Report (EDCR) P-S240, and Nonconformance and Disposition Report (N&DR) FQC-5-9023 were discussed with the licensee representative. The design mix tests and documentation of results have not been completed. Therefore, this item is considered unresolved and will be reviewed during the next inspection.

QA Program Review of Suppliers of Material and Services The IE inspector commenced review of the QA Program for the following suppliers of material and services:

National Mobile Concrete Corporation Delta Testing Laboratories Dixie Sand and Gras el Corporation The QA Manuals and related QA survey documents were not available during the inspection.

This item is considered unresolved and will be reviewed during the next inspection.

Containment Structural Concrete The IE inspector reviewed the Construction Specification 210.350, ADD.3,

" Mixing and Delivering Ccucrete," as well as construction procedures CMP 3.2-12.75, " Concrete Mixing ar.d Delivering," and QS-10.12B, " Mixing and Delivering Concrete." Activities in progress for the preparation for tue placement of Category I concrete 1ere also observed.

results of ASTM C227, " Potential Alkali Reactivity of Cement-Aggregate Combinations (Mortar-Bar Method)." This is a requirement of the Construction Specification.

Fine aggregate from the Thompson Creek Pit does not meet the selection criteria of aggregate material as prescribed in Section 4 of ASIM C227.

Procedure CMP 3.2-12.75 references storage of on-site cement only.

Additional requirements should be included for storage and monitoring of the cement in the off-site storage sites.

These items are considered unresolved and will be reviewed during subsequent inspections.
