On June 30, 2023, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Byron Station. On July 12, 2023, the NRC inspectors discussed the results of this inspection with H. Welt, Site Vice President, and other members of your staff. The results of this inspection are documented in the enclosed report.
No findings or violations of more than minor significance were identified during this inspection.

SUMMARY
The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Byron Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS
Apart from minor reductions in power to support scheduled testing activities or small load changes requested by the transmission dispatcher, Unit 1 remained at or near full power for the entire inspection period.

INSPECTION SCOPES
Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

71152S - Semiannual Trend Problem Identification and Resolution Semiannual Trend Review (Section 03.02)
- (1) The inspectors reviewed the licensee's corrective action program for potential adverse trends associated with rework or early component failure that might be indicative of a more significant safety issue. A specific trend related to certain issues involving corrective action documents associated with potential rework or potentially early component failure is documented in the Results Section of this report.
INSPECTION RESULTS
Observation: Failure of Pressurizer Safety Relief Valve during As-Found Testing 71152A The inspectors performed a detailed review of IR 4561573, 1RY8010C Failed Set-Pressure Test but not IST Limits. The issue occurred when Unit 1 Pressurizer Safety Relief Valve (PSV) 1RY8010C was removed for set pressure verification per the Inservice Testing Program. The valve failed its as-found pressure lift test at the vendor. The PSV lifted at 2408 psig, or 2.1% lower than its nameplate set pressure. Technical Specification (TS) 3.4.10 allows a limit of +/- 2% of the nameplate set pressure (2411-2509) while the ASME Inservice Testing (IST) limit is +/- 3% of the nameplate set pressure (2387-2533). The inspectors chose this issue for an in-depth review of the licensees actions within their corrective action program (CAP) due to previous history of PSVs failing both the TS and IST limits.
Consequently, for this detailed Problem Identification and Resolution inspection sample, the inspectors focused on assessment of the licensees corrective actions taken both in the past and for this present condition.
The inspectors did not identify any issues with the licensees CAP actions taken for past historical issues involving PSVs failing set pressure testing following removal during an outage. These CAP actions were largely focused on updating procedures to improve PSV setpoint stability during the recertification process. Setpoint drift was determined to be the cause of the failures, so the planned changes are structured to reduce the likelihood of large setpoint drift while installed. Similarly, the inspectors review of the licensees current corrective actions also did not result in the identification of any issues of note. The incident documented in IR 4561573 occurred prior to implementation of all corrective actions from previous similar instances. As such, the most recent CAP actions acknowledge this and reinforce the procedure enhancements put in place to reduce as-found test failures.
No findings or violations of NRC requirements were identified by the inspectors in the course of this review.
