J. C. Brya'nt, Section C f, RCES Branch Gate Signed SUMMARY Inspection on August 21-24, 1979 Areas Inspected This routine, unannounced inspection involved 27 inspector-hours onsite in the areas of concrete placement, equipment storage, review of CDR's, IEB's and IEC's and drawing controls.
Results Of the five areas inspected. Lo items of noncompliance or deviations were identi-fied in four areas; one item of noncompliance was found in one area (Infraction -
Failure to sample concrete at the point of placement - Details paragraph 5).

Exit Interview The inspection scope and findings were summarized on August 24, 1979 with those persons indicated in Paragraph I above. The licensee was advised of the basis for the apparent infraction; failure to test concrete at the point of placement.

Licensee Action on Previous Inspection Findings (Closed) Infraction 424/79-09-01, Unauthorized placement of rejected con-crete.
The licensee's response to RII dated June 22, 1979 advised that personnel changes had been made and an engineering evaluation of the two batches of concrete in question was to "use as is".
Based on the observa-tions of the inspector during this and a previous inspection, the licensee appears to be in compliance with the method of handling out of tolerance concrete. RII has no further questions at this time.
(0 pen) Deficiency 424-425/79-11-02, Backfill work stoppage. RII has receiv-ed GPC's responses dated July 31, and August 7, 1979.
The corrective action will be evaluated during upcoming inspections.

Independent Inspection Effort A. Concrete Placements The inspector observed the placement of a section of base slab in the Unit 2 auxiliary building. The placement (Pour No. A-080-004C-2)
contained 358 cy of 5000 psi concrete which was pumped to the point of placement. Temperature, slump and air content were measured in accord-ance with the requirements of Procedure CD-T-02, Concrete Quality Control.
An equipment pad No. A-1210-T4-002 (Pour No. A-080-135) placement consisting of 42 cy of 5000 psi concrete was observed. The placement was made using a 3 cy bucket. During this pour the vibrators were used in accordance with the instructions set forth in CD-T-02.
A review of the records for this equipment pad and a similar equipment pad A-1210-T4-001, (Pour No. A-080-153) in Unit I revealed that the slump, air and temperature tests were conducted at the concrete lab near the batch plant and not at the truck discharge to the b.icket.
Construction Procedure CD-T-02 requires that concrete shall be sampled in accordance with ASTM C-172 which states that sampling should be performed as the concrete is delivered to the vehicle conveying the concrete to the forms. This is a noncompliance and is identified as infraction 424/425/79-14-01, Failure to test concrete at the point of placement. A similar item was identified during an inspec-tion conducted November 28-30, 1978 (IE:II Report No. 78-S.)
The inspector observed a placement of concrete for the Unit 1 tendon gallery inner and outer walls (Pour No.1-01D-010). Due to equipment problems only the outer wall was poured consisting of 73 cy.
The following day the inner wall was poured.
Both sections of wall had starter grout applied. W en the equipment problem arose the grout was removed from the inner wall section.
The interruption of the place-ment at the outer wall section did not appear to be long enough to require that a bulkhead be installed as required by Procedure CD-T-02.
This pour will be examined during subsequent inspection to insure that consolidation was accomplished. This will be identified as an Inspec-tor Followup Item 424/79-14-02, Review of tendon gallery pour 1-01D-101.
B.
Pressure vessel storage The inspector examined several tanks stored in the warehouse yard which required nitrogen blankets while in storage. All appeared to be stored adequately.
A review of the inspection records indicated that the pressure in the tanks was checked by the QC inspectors weekly.
If the pressure drops to or below 1 psi the mechanical equipment engineer is notified and corrective action is taken. Wile records indicate that weekly inspec-1311 329

tion is being accomplished, there is no record of QC acceptaA - of the corrective action taken until the next weekly inspection is perrormed.
In discussons with the licensee, the inspector pointed out that this method did not provide verification by QC that purging and repressuri-zation were performed correctly. The licosee agreed and will revise Procedure MD-T-06 to incorporate a QC inspection mport covering these activities. This will be identified as Inspector Followup Items 424/79-14-03 and 425/79-14-02, Inspection of instorage maintenance.
C.
Drawing Control The inspector selected several drawings at random in the field tt, verify that the drawing control methods are assuring that the latest revisions only ere used for construction. No problems were identifi-ed.
The licensee has developed an audit program to insure that only the latest revision of drawings are utilized in the field.
These audits will be performed by the drawing control section personnel.
6.
IE Bulletins (IEB) and IE Circulars (IEC)
(0 pen) IEB 78-12, 12A, and 12B, Atypical weld material in reactor pressure vessel welds. The licensee's response dated June 29, 1979 will be evaluat-ed in IE Headquarters.
(Closed) IEB 78-05, Malfunctioning of circuit breaker euxiliary contact mechanism - General Electric Model CR 105X. The licensee's response dated June 7, 1978 to RII advised that the referenced mechanism would not be used at Plant Vogtle.
(Closed) IEB 78-06, Defective Cutler-Hammer type m relays with DC coils.
The licensee's response to RII dated July 24, 1978 advised that thit type relay would not be used at Plant Vogtle.
(Closed) IEB 78-14, Deterioration of Buna N components in ASCO solenoids.
This bulletin does not apply to Plant Vogtle.
(0 pen) IEB 79-02, Pipe support base plate designs using concrete expansion anchor bolts. The licensee's response dated July 6, 1979 is being evalua-ted.
(Open) IEB 79-03, Longitudinal weld defects in ASME SA-312 type 304 stain-less steel pipe spools manufactured by Youngstown Welding and Engineering Corporation. The licensee submitted the response dated May 9, 1979 to this bulletin with answers to all action items which IE:HQ will evaluate.
(Closed) IEB 79-04, Incorrect weights for swing check valves manufactured by Velan Engineering Corporation. The licensee's responses dated' May 25, 1979 stated that Velan swing check valves would not be used at Plant Vogtle.

(Closed) IEB 79-07, Seismic stress analysis of safety-related piping. The licensee's response dated May 24, 1979 adequately answared the concerns of the bulletin. RII has no further questions at this time.
(Closed) IEB 79-09, Failures of GE type AK-2 circuit breaker in safety-related sysems. The licensee's respcase dated May 29, 1979 stated that this type breaker will not be used at Plant Vogtle.
(Closed) IEB 79-05, 79-06, 79-08,79-05A, 79-06A,79-06B. These bulletins are information only type bulletins relating to TMI. No response was reqtiired.
