Results Of the 4 areas inspected, no apparent items of noncompliance or deviations were identified in 3 areas; 2 apparent items of noncompliance were found in 1 area (Infraction - backfill noisture control, paragraph 5 and Deficiency - backfill work stoppage, paragraph 5).

Exit Interview The inspection scope and findings were summarized on June 14, 1979. The inspector discussed in detail the noncompliances (see paragraph 5) on control of Category I backfill. The licensee stated Category I backfill operations are stopped and will not commence until Bechtel evaluates the compaction controls. The licensee will inform RII:IE prior to resumption of backfill operations.

Licensee Action on Previous Inspection Findings a.

(0 pen) Infraction (79-01-01): Calibration of Fine Aggregate Test Sieves.

Two sets of calibrated sieves have been received from the National Bureau of Standards. A procedure for on site recalibration of test sieves is being developed.

b.

(Closed) Infraction (79-07-02): Electrical preventative Maintenance.

The inspector examined the licensee response dated June 2,1979 and the implementation of the corrective action stated in the response. A computer program is being developed to preclude a recurrence of the noncompliance and will be examined in future inspections. This item is closed.

(0 pen) Deviation (79-07-01) Concrete Design Commitments. The licensee's c.

response has been sent to NRR. This item remains open pending NRR's evaluation of the licensee's response.

d.

(Open) Infraction (79-08-91) Failure to Document Backfill Test Data.

The licensee has developed a procedure and method for documenting moisture test data.

However, the method and procedure which was recommended by Bechtel is inadequate; see paragraph 5.

This item remains open pending resolution of backfill controls.

Independent Inspection The inspector examined construction status and compaction controls on Category I backfill.

Examination of Compaction controls disclosed the following noncompliances:

a.

Procedure CD-T-01 and specification X2AB01 specifies the moisture content of Category I backfill, at the time of placement and shall be controlled to plus or minus two percent of optimum. Test data dated June 5 through June 7,1979 shows that the "Speedie" method that has been used to control moisture content on backfills allows a variation

from optimum of up to plus or minus six percent. Failure to control the moisture content of backfills in accordance with procedure CD-T-01 and specification X2AB01 was identified to the licensee as Infraction 424/425/79-11-01, " Backfill Moisture Control."

b.

Procedure GD-T-07, " Work Stoppage," requires that a "Stop Work Notice"

be issued when work is being performed in violation of governing drawings, specifications, code or approved procedures. A "Stop Work Notice" was not issued on June 5,1979 when testing on backfill moisture content dated June 5 through June 7 showed that the moisture content of backfills could not be controlled to the plus or minus two percent specified in procedure CD-T-01 and specification X2AB01. Failure to issue a Stop Work Notice as required by procedure GD-T-07 was identified to the licensee as Deficiency 424/425/ 79-11-02, " Backfill Work Stoppage."

Foundations - Observation of Work and Work Activities The inepector observed partial placment of pour number 1-05-E-001A in the

bottom mat of the IB Nuclear Service Water Tower. Acceptance criteria examined by the inspector appear in the following documents:
