Areas Inspected:
Concrete placement; CDR's; IEB's; and previous items of noncompliance. The inspection consisted of 20 inspector-hours on site by one NRC inspector.
Results:
Of the four areas inspected, no items of noncompliance or devia-tions were identified in three areas, one item of noncompliance (Inf raction -

Licensee Actions on Previous Inspection Items (Open) Deficiency 424/78-08-01 - Nonconformance Control. The inspector toured the warehouse area to review the handling of noncomforming items. Several valves were noted that had " Hold" tags attached noting the need for documentation. Discussions with the mechanical QC inspec-tors were held. The inspector was advised by the licensee that no valves were issued until the " Nonconforming Log" was checked to insure that the valves were in conformance in all aspects.
This item remains open until the response is received and evaluated.

Independent Inspection Effort The inspector observed concrete placement A-11D-003 in progress. This placement consisted of a total of 216 yards of concrete at the Unit 2 control building tendon gallery access shaft. There were no imbeds in this pour.

During placement, it was noted on two different occasions that the concrete was allowed to accumulate et the end of the conveying pipe to a height of approximately 2 feet above the finish elevation. Vibrators were then used to move the pile of concrete to the required elevation.
GPC Construction Procedure CD-T-02, Concrete Quality Control, section 6.1.9 paragraph 3 states " Vibrators shall not be used for transporting or moving concrete inside the form." This is an infraction identified as 425/78-09-01, Failure to follow procedures during concrete placement.
The inspector observed the pumping operation and the batch plant during this placement. The QC inspector at the batch plant was inter-viewed regarding his duties. The surveillance performed by the inspector is documented in a daily lo3 which is stored in the vault under daily inspection files. Any problems encountered at the batch plant during the day of a placement are entered in the daily log while the other records for the placement are filed by pour cards numbers. The pour card record file and the daily inspection log files must be reviewed in order to review a completed placement record. The licensee is aware of this problem and has identified it as an open item in a QA audit. This will be identified as an inspection followup items 424/
78-09-01 and 425/78-09-02, Consolidation of concrete placement files.
The inspector observed that slump tests, air tests and temperature measurement were being taken at the concrete lab located beside the batch plant. After testing, the concrete was transported approximately one fourth mile to the pumping equipment. The inspector observed no further testing of the concrete.
GPC Construction Procedure CD-T-02 references ACl Standards as applicable. The ACI Standards references ASTM C-172 which makes provisions and requires for taking samples at the point of placement. This is another example of infraction 425/
78-09-01, Failure to follow procedures during concrete placement.
5.
Licensee Identified Items 10 CFR 50.55(e)
(Open) Item 424/78-09-02 and 425/78-09-03, Improper fabrication of imbeds.
The licensee reported a potential CDR regarding the fabrica-tion and installation of imbeds to RII on November 22, 1978. All concrete work involving imbeds had been stopped as of November 21, 1978.
However, on November 29, 1978, two QA auditors touring the auxiliary building found that embeds were being installed.

s RII Report Nos. 50-424/78-9 and 50-425/78-9 I-3 there was a lack of communication between supervision and the field forces. A meeting was held between the project manager and the parties involved and it appears that communication channels have been improved.
This item will be followed by the inspector to insure that the communi-cations channels are functioning and is identified as inspector followup items, 424/78-09-03 and 425/78-09-04 Supervision to field communications.
6.
IE Bulletins and Circulars (IEB & IEC)
(Open) IEB 78-12, Atypical Weld Material In Reactor Vessel Welds. The licensee has contacted the supplier of the reactor vessel regarding this bulletin. An interim answer was planned requesting additional time for response. In the meantime IEE 78-12A was issued which extended the time for response.
7.
Exit Interview The inspector met with the licensee representatives denoted in para-graph I at the close of the inspection. The scope and findings of the inspection in the areas of concrete placement, construction deficiency reports, infractions and IE Bulletins were discussed.
On December 5,1978, the licensee was advised that infraction 425/78-09-01, failure to follow procedures during concrete placement, was issued to replace 2 deficiencies which were cited at the exit interview.
