Licensee Action on Previously Identified Items (Open) Unresolved Item (373/79-11-01):
Cable tray edge softeners not installed in potentially hazardous locations. During this inspection the licensee indicated that a purchase order had just been released to obtain the softeners. The licensee stated that, at those locations where soften-ers must be installed, QC inspections will be made of all the applicable cables to identify any damage.
If any cables are damaged, then appropriate actions will be taken.
(Closed) Noncompliance (373/79-11-05): The minimum cable bend radius was not maintained for more than 16 cables. Reference IE Reports 79-13 and 79-21.
During this inspection, the RIII inspector verified that the 22 cables in question were supplied by Okonite. During a phone conversation with the licensee on January 31, 1980, the cognizant Okonite engineer indicated that documentation substantiating the cable bend radius require-ments for cable not in tension would be forwarded to the site. On February 13, 1980, the licensee told the RIII inspector that the Okonite documentation had been received and was acceptable. The two cables (#1RH190 and #1VD02) which also violated the new bend radius criteria, were documented as pulled out.
H. P. Foley Instruction No. HPFCo-WI-500 Revision 8 indicates the new static (not in tension) bend radius re-quirements for Okonite cables and also references the original bend radius requirements in WI-400 for cables from all other manufacturers.
(Closed) Noncompliance (373/79-11-06): Failure to assure that maximum cable pull tensions were not being exceeded. Reference IE Report 79-21.
Reanalysis by CECO indicated that only four of the eight cables, origin-ally identified, had potentially exceeded tension requirements. The RIII inspector reviewed CECO Nonconformance Report, NCR, #318. The NCR in-

dicated that the four suspect cables had been repulled through the perti-nent conduit sections. The repull activities indicated that the maximum pulling tensions of the four cables had not been and were not exceeded.
H. P. Foley Company Instruction HPFCo-WI-400, Revision 6, paragraph 3.3.12 has been written to require tension measurements for cable pulls involving over 270 degrees in composite bends.
(Closed) Unresolved Item (373/79-11-07): QC inspection of cable instal-lation activities in a timely manner. This item was actually closed in IE Report No. 79-21.
This item was misnumbered as 373/79-11-08 in Report No. 79-21.
(Closed) Noncompliance (373/79-13-01): No procedural requirements for QC verification of electrical cable stress cones.
During this inspection, the licensee provided inspection reports which indicated that 21% of the stress cones question were reinspected.
Only a few minor discrepancies were identified on some of the reinspected cones. The licensee stated that when a discrepancy was identified, then the stress cones on all three phases of the applicable cables were redone. The reworked stress cones have been and all future stress cones will be verified by QC to be in accordance with the requirements of HPFCo-WI-500 Revision 8.
(0 pen) Unresolved Item (373/79-13-03):
Site system turnover program.
The RIII inspector reviewed CECO Audit Raport No. 1-79-35 performed May 15, 1979 through June 27, 1979. The audit identified significant findings which have not yet been resolved.
The RIII inspector discussed the findings and the program which CECO intends to implement to assure a comprehensive and effective systems turnover.
The licensee indicated that the proposed program will be modified to include a complete QC records review to assure accurate assessment of construction status at turnover (pre-operational testing and fuel loading). This item remains open until the resolutions to the audit findings are controlled and the above turnover program is effectively implemented.
(0 pen) Followup Item (373/79-13-06):
Incomplete procurement documentation for all safety related components (electrical, mechanical, etc.).
During this inspection, the program, to identify documentation shortages, was discussed. The program essentially consists of two parts:
(1) Admini-strative or specification specific and (2) component specific. The administrative program will identify the documentation shortages which are a one time submittal. The component specific program will identify the documentation shortage which are not a one time submittal. To date, the licensee has reviewed approximately 6000 of the 7000 existing Material Receiving Reports, (MRRs) to identify the administrative documentation shortages. The administrative review should be current with the MRR's by April 1, 1980. The component review program will be fully active to identify the component specific documentation shortages by May 1,1980.
This item remains open until all of the procurement documentation is made available to the NRC, the NRC reviews are completed, and the discrepancies identified in IE Report 79-13 (page 10) are resolved.

(0 pen) Unresolved Item (373/79-13-07): Environmental qualification test results for kerite cable splicing and terminating kits. The licensee stated that the documentation was currently being reviewed by Sargent &
Lundy. This item remains open until the licensee completes their review and the NRC reviews the documentation.
(0 pen) Noncompliance (373/79-13-09; 374/79-10-01):
Redundant post ac-cident tracking circuits for the reactor pressure / level were incorrectly classified as nonsafety related. During this inspection the RIII in-spector reviewed the purchase order which indicated that the recorders (located in the control room) will have seismic qualification.
The pertinent cables have been upgraded to Class 1E as identified on Foley Nonconformance Report No. 180. This item remains open pending an NRC assessment of CECO's detailed review of Sections 7.5 and 7.6 of the FSAR.
Reference CECO's supplemental response dated September 5, 1979.
(Open) Noncompliance (373/79-13-17): Audits not performed to verify the installation of NSSS instrument panels. The RIII inspector reviewed CECO Audit #1-79-39 dated May 31, June 1, and June 4, 1979. Audit Finding #8 addresses problems with the installation of instrument panels.
Corrective action, though in process, has not been completed.
This item remains open until the concerns, addressed in Finding #8, are adequately disposi-tioned and controlled.
(0 pen) Followup Item (373/79-21-01):
Installation of 4160 volt motors.
The licensee indicated that one of the Resident Heat Removal (RHR) pump motors was scheduled for installation during February 2-4, 1980. The remaining two RHR and LPCS (Low Pressure Core Spray) pump motors are scheduled to arrive in mid-May, 1980.
The motor installations will be reviewed during a subsequent inspection.
(Closed) Unresolved Item (373/79-21-02):
Incomplete installation QC documentation for standby and High Pressure Core Spray diesel generators.
During the IE Report No. 79-36 inspection period, the RIII inspector re-viewed manufacturer's QC inspection documentation indicating that the installation criteria had been inspected by the manufacturer's QC repre-sentative.

Specific raceway loading (fill)
conditions. Reference IE Report No. 79-39.
The licensee site personnel confirmed that surveillances are in process to verify associated cable routings. The licensee also indicated that these surveillances would not be completed for some time because of the large number of cables involved.

Electrical Records The RIII inspector made a brief review of the QC installation records for components in the 250 volt d.c. system.
No items of noncompliance or deviations were identified.

Observation of Electrical Testing The RIII inspector observed part of a rated voltage test on the three phases of the nonsegrated bus connecting Switchgear 242Y (blue) to the System Auxiliary Transformer #242. The test was performed to substantiate the bus integrity with the lightning arrestors connected.
No items of noncompliance were identified.
Exit Interview The inspector met with licensee representatives on February 1, 1980. The inspector summarized the scope and findings of the inspection. The licensee acknowledged the findings as reported.
