Inspection Summary Inspection on January 17-18, 1980 (Report No. 50-373/80-03; 50-374/80-02)

Areas Inspected:

Status of safety related portions of the heating, venti-i lating, and air conditioning (HVAC) system.

Nonconfornance reports and the resolution of these items. Reviewed the documentation and observed

the repairs of the non-segregated 4KV bus to HPCS switchgear 243 from system aux transformer 242 at 710' elevation. The inspection involved 16 inspector-hours by one NRC inspector.

Results: No items of noncompliance or deviations were identified.

Functional or Program Areas Inspected Details of functional or program areas inspected are documented as follows.

1.

Correction to Report Nos. 50-373/79-43; 50-374/79-28 The statement at the top of page 5, Item 4 in the Details Section erroneously reports the Unit 2 HVAC systems required to be complete for operation of Unit 1.

This status correctly represents the status of completion for Unit 2 systems but erroneously reports the Unit 2 systems required to be completed and tested prior to operation of Unit 1: These systems will be specified by the licensee as start-up procedures are written and approved.

Heating, Ventilation, and Air Conditioning (HVAC)

The inspector concentrated his attention on the Unit 1 HVAC system since a large portion of the systems have been transferred to the Operating Department. The following HVAC systems have been trans-ferred from the Construction Department to the Operations Department:

a.

Off gas Filter Building HVAC System, System Identification Code (VO).

Common to Units 1 and 2.

b.

Diesel-Generator Room Ventilation System (VD).

CSCS Equipment Area Cooling System (VY).

Turbine Building Ventilation (VT) System.

Service Building HVAC systems 1 and 2 (VS).

Machine Shop Ventilation System, common to Units 1 and 2 (VJ).

Lake Screen House Ventilation System for both Units 1 and 2, (VH).

h.

Switch Gear Rooms Ventilation Systems, (VX), except for the re-circulation duct in the Auxiliary Building IIVAC Equipment.

i.

Auxiliary Building Laboratory HVAC System (VL) common to Units I and 2.

System shows 85% complete on Control Room HVAC, VS, and the Auxiliary Electric Equipment Room (VE).

j.

The A Trains are complete for both of these. The B trains are scheduled for completion in March, 1980.

k.

The miscellaneous Ventilation System (VV), and the Reactor Building Ventilation System (VR) are scheduled for transfer to the operations in February,1980.

The following systems are scheduled for transfer to Operations during March, 1980.

(1) Auxiliary Building Office HVAC Systems common to Units 1 and 2 (VA).

(2) Primary Containment Ventilation System (VP).

(3) Primary Containment Purge System for Units 1 and 2 (VQ).

(4) Reactor Building Ventilation System Units 1 and 2 (VR).

(5) Control Room HVAC System (VC) and Auxiliary Electric Equipment Room (VE), the B train for each.

(6) Diesel Generator Room Unit 2 (VD), swing diesel.

3.

Ground Fault in 4KV Non-Segregated Bus The information concerning this incident was documented on noncomfor-mance report No. 388. dated January 18, 1980.

It was described as a short to ground of the middle phase of 4KV bus to HPCS switchgear 243-3-

from system Auxiliary transformer 242. The short was caused by a

scaffold joint pin being inserted through a vent screen hole in the bua cover.

The short caused a pit approximately one inch in diameter and 1/16 inch deep. The metal from the pit spattered on the adjacent phases.

Three cross braces were bowed approximately one to three inches outward.

The repair of the 4KV bus consisted of the following:

a.

Pitted area approximately one inch in diameter, 1/16 inch deep was ground out and filled in with aluminum and ground smooth.

The work was performed by a certified welder in accordance with an approved welding procedure.

b.

Aluminum splatter was removed from all three phases by grinding smooth, c.

Repair areas were penetrant tested by Conam and found to be acceptable d.

Three cross braces were straightened.

e.

Bus duct was recoated with manufacturer's supplied coative As a measure to prevent such incidents in the future, the licensee has had the screen vents plugged with a metal plate in areas access-able for future scaffold erection.

4.

Nonconformance Reports for !!VAC System The inspector initiated a review of the nonconformance reports for Zack Company. At the date of the inspection, 55 NCR's had been written on the HVAC systems and 26 of these remained open. Three remain open from 1978 and 17 from 1979.

The major number of those open concern hanger placement or interference. The Zack Company has recently received authorization to use 7018 weld rod.

Several of these NCRS concern welding; therefore they will be reviewed when the work is more nearly complete.

The inspector met with licensee representatives (denoted under Persons Contacted) on January 18, 1980. The inspector summarized the scope and findings of the inspection. The licensee acknowledged the findings as reported.
