Inspection on April 29-30, 1980 (Report No. 50-352/80-07)
. Area Inspected:
Routine, unannounced inspection by one regional based inspector of rework / modifications on reactor pressure vessel internals.
The inspection involved 14 inspector hours onsite by one regional based inspector.
Resul ts:
Of the one area inspected, one item of noncompliance was identified.
(Infraction - Failure to provide adequate procedures).

2.
Modification of Reactor Pressure Vessel Internals The inspector audited the modification / rework activities being performed onsite by Reactor Controls Incorporated.
The purpose of the audit was to ascertain compliance with the PSAR and General Electric document 22A4202 Rev. 5.
The following documentation was reviewed.
r Purchase Order 205/A310 Rev. O from G.E. to Reactor Controls

Incorporated dated March 13, 1980. The purchase order required Reactor. Controls to perform modification / rework on the Limerick 1 Shroud, Shroud Head, Top Guide, Steam Dryer, and Shroud Backing Ring.
Technical Specification, Rev. 1 dated March 1, 1980 which states

-that all welding, cleaning, NDE and quality requirements shall be per G.E. document 22A4202, Revision 5.

Visual observation of work activities.

During the visual observation of the shroud the inspector noted 11auid penetrant dyes in crevices which would not easily be removable.
The contractor, Reactor Controls, had no procedures applicable to control the grinding.. liouid penetrant and preventive measures regarding dye contaminants. entrapment.
The inspector found'that G.E. document 22A4202 contains specific instructions regarding penetrant entrapment by stating:
" Measures shall be taken to prevent entrapment of penetrant materials in crevices."
The GE FDI 52-73030 applicable to the work activity also stated: "PT inspection shall be done after all grinding to within b" of any open crevices."
Failure to have adequate procedures to prevent penetrant entrapment is contrary to 10 CFR 50 Appendix B, Criterion V.
This item is an infraction (352/80-07-01).
