On July 11, 1972, during a routine site inspection, Mr. C. M. Erb was informed of the event as follows:

Description of Event:

The pump volute, Serial No. 701N0241 (weighing 12 tons) was shipped from Byron-Jackson, Los Angeles, California, on May 30, 1972, and arrived at the site June 15, 1972. While enroute by C&H Transportation Company, an accident near Tulsa, Oklahoma, caused the volute to roll off the truck.

The inspector examined the volute and found damage to the dis-charge nozzle and also to the top seal surface where the motor assembly is to be attached. During the inspection, a Toledo Edison QA engineer photographed the damage and gave the inspector a set of three pictures which are on file in the Region III office.

The volute was procured through, and source inspected by, Babcock and Wilcox.

Action by Licensee:

Present plans are to ship the volute back to Byron-Jackson for inspection and any required repair.
