L-97-322AttachmentP~g1~ViolaioATechnicalSpecification3.6.2.1statesthattwocontainmentspraytrainsandtwocontainmentcoolingtrainsshallbeOPERABLE.Contrarytotheabove,betweenMay16,1997andOctober8,1997,whileoperatinginModes1,2,and3,oneUnit2containmentcoolingtrainwasinoperable.R~g~ny1.FPLconcurswiththeviolation.2.REASONFORVIOLATIONThisviolationwascausedbyboth:a)personnelerrorduringpostmaintenancetestingactivitiesassociatedwithcontainmentcoolingfan2-HVS-1D;and,b)aninadequate31dayTechnicalSpecificationsurveillanceprocedure.Thesedeficienciescontributedtoreturningcontainmentcoolingfanunit2-HVS-1Dtoservicewhenitwasunabletoperformitsemergencymodeofoperationduetoreverseslowspeedoperation.CausesandcorrectiveactionsassociatedwiththeabovearediscussedinFPL'sresponsestoviolationsBandC,respectively.Thelowflowalarmfor2-HVS-1Dwasnotdocumentedduringtheperiodic18monthtestoftheengineeredsafetyfeaturesbecausethecontainmentfancoolerunitwasoutofserviceduringthetest.Althoughtheproceduralrequirementsfordocumentingactionstobetakentoresolveannunciatorsreceivedduringtheperformanceofsafeguardstestingwereweak,thistestwasnotamissedopportunityfordiscoveringthat2-HVS-1Dwasinoperableduringtheemergencymodeofoperation.3.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDFPLenteredtheapplicableTechnicalSpecificationACTIONstatementswhenthelowflowalarmswerereceivedduringinitialandsubsequentcontainmentfancoolerslowspeedtestingonOctober6andOctober7,1997.Thereverseslowspeedoperationwascorrectedbyimplementingatemporarysystemalterationthatreversedtwoofthethreepowerleadstothelowspeedwindingofthefanmotoratthemotorcontrolcenter.Thisactionwascompletedwithintheallowedoutagetimeforinoperablecontainmentfancooler(s).Containmentfancoolerunit2-HVS-1DwasreturnedtoserviceonOctober9,1997,afterproperoperationoftheunitwasverifie L-97-322AttachmentP~ae24.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSFPLrevisedsurveillanceoperatingprocedure1-0400050,"PeriodicTestoftheEngineeredSafetyFeatures,"toenhancethedispositionofannunciatorsthatarereceivedduringtheperformanceoftheIntegratedSafeguardsTest.Thisrevisionincludesanewappendixusedtotrackanddispositionannunciatorsthatoccurduringthetest.ThisrevisiontotheUnit1procedurewascompletedonNovember18,1997.B.TheUnit2Safeguardsprocedure,2-0400050,willberevisedtoincludethenewannunciatorresponseappendixpriortoperformingsafeguardstestingduringtheUnit21998refuelingoutage(SL2-11).FullcompliancewasachievedonOctober9,1997,whencontainmentfancoolerunit2-HVS-IDwasreturnedtoservic L-97-322AttachmentP~ae3VIolation810CFR50,AppendixB,CriterionV,Instructions,ProceduresandDrawings,requires,inpart,thatactivitiesaffectingqualityshallbeprescribedbydocumentedproceduresofatypeappropriatetothecircumstancesandshallbeaccomplishedinaccordancewiththoseprocedures.MaintenanceProcedure(MP)2-0950173,Revision9,"TheOverhaulofContainmentFanCoolersMotors2HVS-lA,1B,1C,1D(EQMotors),"Section9.9,implementsthepostmaintenancechecksofthecontainmentfancoolermotorsaftermaintenance.MP2-0950173requiresthefantobestartedinslowandfastspeedtoverifyproperbladerotationineachspeed.Contrarytotheabove,onMay16,1997,thelicenseefailedtoperformSection9.9ofMaintenanceProcedure2-0950173properlyinthatcontainmentfancooler2-HVS-1Dwasnotstartedinslowspeedandproperbladerotationwasnotverified.~Reaone1.FPLconcurswiththeviolation.REASONFORVIOLATIONThecausefortheviolationwaspersonnelerrorthatoccurredduringtheexecutionofpostmaintenancetestingperformedduringthelastUnit2refuelingoutage.Thiserrorresultedinreverserotationofcontainmentfancooler2-HVS-1D.TherootcauseevaluationconcludedthattheimpropertestexecutiononMay16,1997,wascausedbyfaultycommunicationsbetweenthemaintenancepersonnelassessingfanrotationinsidecontainmentandthecontrolroomoperatorwhostartedthefan.FPLreviewedthecompleteddocumentationfortheMay16,1997,postmaintenancetestandcompareditagainsttheMay16,1997,Unit2controlroomsequenceofeventsrecorder(SER).TheSERshowedthat2-HVS-1Dwasstartedtwiceduringthepostmaintenancetesting;however,boththeslowandfastspeedrotationalcheckswereperformedwiththemotoroperatinginfastspeed.Thefactthattwomotorstartsweredoneimpliesthatmaintenancepersonnelwerecorrectlyfollowingthetestproceduresteps.Theproperuseofthreewaycommunications,asrequiredbyboththeConductofMaintenanceandConductofOperationspolicies,wouldhavepreventedthefaultycommunicationwhichledtotheunsatisfactoryexecutionofthepostmaintenancetes L-97-322AttachmentP~ae43.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDA.ProcedureMP-2-0950173,"TheOverhaulofContainmentFanCoolerMotors2-HVS-lA,1B,1C,1D(EQMotors),"wasrevisedtorequirethefollowing:1)verificationsignaturesforbothfastandslowspeedmotorrotationtesting;2)acautionstatementtoensurethatbothwindingsareenergizedtoverifypropermotorrotationsubsequenttoelectricallyreconnectingthemotor;3)anaddendumsheetthatwillbeusedbythecontrolroomoperatorstofollowtheperformanceofthemotorrotationchecks;and,4)arequirementtoperformafunctionaltestofthecontainmentfancoolerinbothfastandslowspeedpriortoreturningtheunittoservice(i.e.,nolowflowalarmspresentduringrotationchecks).TheprocedurerevisionswerecompletedonNovember28,1997.AdditionaleditorialclarificationsweresubsequentlyaddedandapprovedonDecember17,1997.4.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSA.Thisspecificevent,includingtheimportanceofaccuratecommunicationsandtheuseofthreewaycommunications,wasincludedinaMaintenancetrainingbulletinthatwasissuedonNovember22,1997.B.ProcedureADM-78.01,"PostMaintenanceTesting,"willberevisedtoaddacautionnoteabouttestingmultiplespeedormultiplewindingmotorsbyJanuary31,1998.5.FullcompliancewasachievedonNovember28,1997,whentheprocedurerevisionstoMP-2-0950173,"TheOverhaulofContainmentFanCoolerMotors2-HVS-1A,1B,1C,1D(EQMotors),"werecomplete L-97-322AttachmentP~s5ViolationC10CFR50,AppendixB,CriterionXI,TestControl,requires,inpart,thatatestprogramshallbeestablishedtoassurethatalltestingrequiredtodemonstratethatcomponentswillperformsatisfactorilyinserviceisidentifiedandperformedinaccordancewithwrittentestprocedurewhichincorporatetherequirementsandacceptancelimitscontainedintheapplicabledesigndocuments.FinalSafetyAnalysisReportSection6.2.2.2.2statesthatthefancoolersoperateatareducedflowof39,600cubicfeetperminuteeach,withlowspeedmotoroperationduringaccidentoperation.NormalOperatingProcedure,NOP-2-2000020,Revision2,"ContainmentCoolingSystemOperation,"whichimplementsmonthlysurveillancetestingofthecontainmentfancoolers,requiresthateachcoolingtrainfanunitbestartedfromthecontrolroomandverifiedthateachunitoperatesforatleast15minutes,atleastonceper31days.Contrarytotheabove,asofOctober6,1997,thelicenseefailedtoassurethatalltestingrequiredtodemonstratethatcomponentswillperformsatisfactorilyinservice,inaccordancewiththerequirementsandacceptancelimitscontainedintheapplicabledesigndocuments,wasidentifiedandperformed.Specifically,NOP-2-2000020,didnotrequirethecontainmentcoolerfanstobetestedinslowspeed,whichisthealignmentrequiredduringemergencyoperation.Thisproceduralinadequacyhadexistedsinceinitialplantoperation.R~esense1.FPLconcurswiththeviolation.2.REASONFORVIOLATIONThecausefortheviolationwasnon-cognitivepersonnelerrorduringthedevelopmentofsurveillanceprocedureNOP2-2000020,"ContainmentCoolingSystemOperation,"thatresultedinthesurveillanceproceduredeficiency.Theprocedure,asitwaswritten,didnotmeettheintentoftheTechnicalSpecificationsurveillancerequirementsbecauseitrequiredtestingofthecontainmentcoolerfansonlyinthefastspeed,anddidnotrequireverificationofthepost-accidentsafetyrelatedfunctionofthefansinslowspee L-97-322AttachmentP~ae63.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDProcedureNOP2-2000020,"ContainmentCoolingSystemOperation,"wasrevisedandissuedonOctober13,1997.Therevisionclearlyrequiresthatallfansmustbestartedandruninslowspeed.Onceallfourfanshavebeenshiftedtoslowspeed,theabsenceofanylowflowalarmsthroughoutthe15minuteruntimeconstitutessatisfactorycompletionofthesurveillancetest.Theprocedurealsorequiressatisfactoryfastspeedoperationofthecontainmentfancoolers.B..FPLinvestigatedthegenericimplicationsofthiseventtodeterminewhatotherequipmenttypesmaybesubjecttoinadequatesurveillances.Thereviewrevealedthat,withtheexceptionofheating,ventilating,andairconditioning(HVAC)equipment,additionalrequirements(e.g.,ISUISTrequirementsandindustrystandards)existwhichenhancethedevelopmentofsurveillanceprocedures.AllHVACproceduresusedtoperformTechnicalSpecificationsurveillanceswerereviewedtoensuretheintentoftheTechnicalSpecificationsurveillanceweremet.ThisreviewwascompletedonNovember24,1997.4.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSFPLQualityInstruction(QI)QI-S-PSL-1,"Preparation,Revision,Review/ApprovalofProcedures,"Section5.5requiresthatsiteproceduresbeperiodicallyreviewed.ThisQIwasrevisedtorequireaqualifiedindependentreviewofproceduresthataFectsurveillancesorteststoensurethattheprocedureadequatelyimplementstheintentofUpdatedFinalSafetyAnalysisReport(UFSAR)surveillancedescriptionsandTechnicalSpecificationsurveillancerequirementswhenprocedureperiodicreviewsareconducted.ThisrevisionwasapprovedonNovember28,1997.5.FullcompliancewasachievedonOctober13,1997,whentherevisiontoprocedureNOP2-2000020,"ContainmentCoolingSystemOperation,"wasapproved.
