'I,L-97-319AttachmentPug&10CFR50.120(b)(1)requires,inpart,thattrainingprogramsforinstrumentandcontroltechnicians,mechanicalmain'tenancepersonnel,andelectricalmaintenancepersonnelbeestablished,implementedandmaintainedusingasystemsapproachtotrainingasdefine'din10CFR55.4,including,inpart,anevaluationoftraineemasterybasedonlearningobjectivesdescribingdesiredtaskperformance.10CFR50.120(b)(2)requires,inpart,sufficientrecordsoftraineeevaluation,includingtraineemasteryofdesiredtaskperformance,bemaintainedbythelicenseetomaintainprogramintegrityandthatsufficientrecordsbeavailableforNRCinspectiontoverifytheadequacyoftheprogram.Contrarytotheabove,asofOctober3,1997,ADM-08.02.Revision15,"ConductofMaintenance,"wasinadequateinthatitfailedtoimplementasystemsapproachtotrainingforelectrical,mechanicalandinstrumentandcontrolpersonnelbyallowingtheevaluationoftraineemasterywithoutestablishedspecificationsandobjective-basedevaluationcriteriaasrequiredbythesystemsapproachtotraining.Inaddition,ADM-08.02,Revision15,"ConductofMaintenance,"didnotcontaintheprovisionfordocumentingtaskqualificationevaluationsasrequiredbyasystemsapproachtotraining,ThisisaSeverityLevelIVviolation(SupplementI).1.FPLconcurswiththeviolation.2.ThecauseoftheviolationwasaweaknessinadministrativeprocedureADM08.02,ConductofMaintenance,whichcouldhavepermitteddefactojobtaskqualification,inlieuoffullqualification,undertheestablishedsystematicapproachtotrainingforMaintenancejobtaskqualification.ItshouldbenotedthatadministrativeprocedureADM08.02wasnotusedtogoverntrainingandevaluationfunctionsofthesystematicapproachtotraining.Thosefunctionshavebeen,andremain,theresponsibilityoftheSt.LuciePlantaccreditedtrainingprograms,Toassignworkintheplant,supervisors,foremen,chiefs,andleadsuseaqualificationlist,providedbytheMaintenanceTrainingorganization,thatidentifiesthequalificationlevelofsitejourneymen.Thelistisprovidedsothatpersonsresponsibleforassigningworkarenotrequiredtoreviewmasteryoftrainingobjectivesforeachjob.Additionally,plantprocedureADM08.02providedfortheapplicationofsupervisoryoversight,proceduraldirection,orworkorderdetail, L-97-319AttachmentPaga2asappropriate,toallowpersonswhowerenotjobtaskqualifiedtoperformworkwithintheirjourneymanqualifications.PlantprocedureADM08.02wasneverintendedtoprovidedirectionforevaluatingtraineemasteryofobjectivesortocontainrequirementsfordocumentingtaskqualificationevaluations,asrequiredinasystematicapproachtotraining.Arecordsreviewofmaintenancetasksperformedandotherdocumentation(e.g.,ConditionReportsandLicenseeEventReports),inadditiontointerviewswithMaintenancesupervisors,revealednoevidenceofcircumventionofthesystematicapproachtotraining-basedtrainingandqualificationrequirements.FPLacknowledgesthatprocedureADM08.02wasweakinitsdefinitionof"supervisoryoversight."TherequirementtodocumenttaskqualificationevaluationsisimplementedbyplantQualityInstructionsandbylowertierplantprocedures.PlantprocedureADM08.02wasnotintendedtosatisfytheevaluationandqualificationrequirementsof10CFR50.120orfordocumentingtaskqualificationevaluations.Trainingrecords,includingrecordsofwrittenexaminationsandtaskperformanceevaluations,areproperlymaintainedforeachtrainingprogramparticipantatSt.LuciePlant.TheserecordsprovidethebasisforthejourneymanqualificationlistswhichareprovidedtoMaintenancesupervisors,foremen,chiefs,andleads.3.A.Inresponsetoaself-identifiedneedforadditionalclarificationonassignmentofworkanduseofthequalificationlistspriortoworkassignment,theMaintenanceDepartmentimplementedanewsetofMaintenanceffrainingPolicies.Thesepolicies,amongotherthings,reinforced:1)therequirementthatpersonnelmustbequalifiedforassignc-'..sks,and;2)thereviewanduseofqualificationlistsbyfieldsupervisorspriortoassignmentofwork.MaintenanceDepartmentheads,supervisors,foremen,chiefs,andleadswereprovidedtrainingonthesepolicies.Asaresult,Maintenancepersonnelarenotbeingassignedtoperformworkforwhichtheydonothavespecifictaskqualificationsunlesstheyareworkingunderthedirectionofaqualifiedperson.0B.InresponsetoanFPLQualityAssuranceDepartmentauditfinding,onSeptember30,1997,theSt.LuciePlantGeneralManagerissuedaninterimcontrolsletterthatclearlystatedmanagementexpectationsthatpersonnelaretobejobtaskqualifiedpriortoperformingworkindependently.TheSeptember30,1997,letterfocusedonworkassignmentforbothplantandnon-plantpersonnelandidentifiedthataprocedureaddressingthissubjectwouldbeimplemented(subsequentlyimplementedasarevisiontoplantprocedureADM08.02).

I97-319AttachmentPaga34.A.AdministrativeprocedureADM08.02willberevisedtoremovethelanguagethatcouldhaveallowedpersonneltobeassignedworkbasedonsupervisoryoversight.Therevisedprocedurespecificallywillrequirethatpersonnelwhohavenotbeenjobtaskqualifiedshallworkunderthedirectionofqualifiedpersonnelwhenperformingplantmaintenance.TheprocedurewillbeissuedforusebyJanuary23,1998,followingcompletionofpre-implementationtraining.B.Anewadministrativeprocedure,ADM22.01,VerificationofTraining/CertificationforTemporarilyEmployedPersonnel,hasbeendevelopedtoprovideguidanceonthereviewoftrainingandqualificationsfortemporarilyassignedpersonnel,suchascontractors,employeesloanedfromotherdepartmentsduringoutages,andforuprelievingpersonnelduringperiodssuchasvacationsorillness,asrequired.Thisprocedureistheresultofextensivebenchmarkingthroughouttheindustryandrepresentsbestindustrypracticeforscreeningandcertifyingthattemporarilyemployedpersonnelmeetplantstandardsforqualificationonspecifictaskspriortoworkassignment.AdministrativeprocedureADM22.01wasrecommendedforapprovalbytheFacilityReviewGroupandwillbeimplementedinJanuary1998afterappropriatepre-implementationtraining.C.TheMaintenanceSupervisorTrainingProgramwillberevisedtoreinforcethechangesidentifiedinADM08.02andthenewlyissuedADM22.01,aswellastoensurethatmanagementpolicyonuseofthequalificationlistsinassigningworkisfullyunderstoodbytheparticipants.ThattrainingwillberevisedbyFebruary28,1998(priortothenextclassfornewsupervisors).Intheinterim,thisinformationwillbeprovidedtoincumbentsupervisorsascontinuingtraining.5.FullcompliancewillbeachievedbyJanuary23,1998,withthecompletionofitem4.A.,above.
