L-96-197Attathmen<Page1VIOLATIONDuringanNRCinspectionconductedonMay12throughJune8,1996,aviolationofNRCrequirementswasidentified.Inaccordancewiththe"GeneralStatementofPolicyandProcedureforNRCEnforcementActions,"(60FR34381;June30,1995),theviolationislistedbelow:A.TechnicalSpecification6.8.1.arequiresthatwrittenproceduresbeestablished,implemented,andmaintainedcoveringtheactivitiesrecommendedinAppendixAofRegulatoryGuide1.33,Rev2,February,1978.AppendixA,paragraph1.dincludesadministrativeproceduresforproceduraladherence.QI5-PR/PSL-1,Rev68,"Preparation,Revision,Review/ApprovalofProcedures,"Section5.13.1,statesthatallproceduresshallbestrictlyadheredto.Step8.1.28ofprocedureOP1-2200050B,Rev26,"1BEmergencyDieselGeneratorPeriodicTestandGeneralOperatingInstructions"requiresthelicenseeto"completedatasheet2afterdieselgeneratorreachesfullloadandevery30minutesthereafter".Datasheet2identifiedthecoolingwateroutlettemperatureasoneoftheparametersthatshouldbetaken.Contrarytotheabove,onJune6,1996,thelicenseefailedtorecordcoolingwateroutlettemperatureeverythirtyminutesasrequiredbyOP-2200050B.Fullloadconditionswerereachedat5:30a.m.onJune6,1996'peratorsbegantakingthereadingsat10:30a.m.afterthisfailuretofollowOP-2200050BwasidentifiedbytheNRC.ThisisaSeverityLevelIVviolation(SupplementI)RESPONSE:1.FPLdeniestheviolation,ascited.Thebasisfordenyingtheviolationisdiscussedinitem2,belo L-96-197Attaehmen4Page22.BasisforDenintheViolationReirementsAsdiscussedinNRCInspectionReportNo.50-335/96-08,Step8.1.28ofOP1-2200050B,2BEmergencyDieselGeneratorPeriodicTestAndGeneralOperatingInstructions,requiresthecoolingwateroutlettemperaturesberecordedevery30minutesafterthedieselgeneratorreachesfullloadconditions.TechnicalSpecification6.8.l.arequiresthatwritten'proceduresbeestablished,implemented,andmaintainedcoveringtheactivitiesrecommendedinAppendixAofRegulatoryGuide1.33,Rev2,February,1978.AppendixA,paragraphl.dincludesadministrativeproceduresforproceduraladherence.St.LuciePlantQualityInstructionQI5-PR/PSL-1,Revision68,Preparation,Revision,Review/ApprovalofProcedures,Section5.13.1,statesthatallproceduresshallbestrictlyfollowed.St.LuciePlantAdministrativeProcedure(AP)No.0010120,ConductofOperations,Revision83,states,inAppendixD:Logentriesshallbeverifiedtobewithinthe'minimumandmaximumacceptancevaluesfortheparameter.Anexplanationshallberecordedforentriesthatexceedtheminimumormaximumacceptancevalues.DiscussionThe24hourperiodicsurveillancetestofthe1BEmergencyDieselGenerator(EDG)wasbeingperformedperOP1-2200050B.Thetestwasbegunatabout0530onJune6,1996,andthefullelectricalloadingconditionwasreachedshortlythereafter.InstrumentTI-59-015Bwasinoperableatthestartofthe1BEDGsurveillancetest.UponbeingnotifiedbytheSeniorNuclearPlantOperator(SNPO)thatinstrumentTI-59-015Bwasinoperable,theSt.LucieUnit1AssistantNuclearPlantSupervisor(ANPS)decidedtocontinuethesurveillancetestwithoutthereadingsfrominstrumentTI-59-015Bandgeneratedanuclearplantworkorder(NPWO)toreplacethegage.ThedecisiontocontinuethesurveillancetestwithoutthedatafrominstrumentTI-59-015Bwasmadewiththeacknowledgmentthatnothavingthedatafromthatinstrumentwouldnot,byitself,renderthe1BEDG24hoursurveillancetestunsatisfactory.Uponrecordingthehalf-hourlylogreadingsforthe1BEDG,theSNPOresponsiblefortheselogreadingsproperlyannotated,onDataSheet21BDieselAndGeneratorofOPNo.1-2200050B,,thatgageTI-59-015Bwasbrokenandthataplantworkorderhadbeen L-96-197AttachmentPage3written.Half-hourly,logreadingswereconsistentlyannotatedassuch,asrequiredbyAPNo.0010120,untilanalternateinstrumentforrecordingdatapointCoolingPlaterOutletTemperature'FwasidentifiedtotheSNPObytheUnit1controlroomat1030onJune6,1996.Analternatedatapointwasrecorded,andindicatedassuchonDataSheet2,forthebalanceofthe24hoursurveillancetest.Su~arurPPLconcludesthat:1)DataSheet2ofprocedureOP1-2200050BidentifiesthecoolingwateroutlettemperaturefrominstrumentTX-59-015BasoneoftheparametersrequiredtoberecordedduringtheconductofOP1-2200050B.2)TheSNPOdidnotrecordcoolingwateroutlettemperatureeverythirtyminutesasrequiredbyOP1-2200050B.However,sincetheparameterwasoutsideofitsminimum/maximumacceptancevalue,theSNPOproperlyprovidedanexplanationateachhalf-hourlyreadingonDataSheet2ofOP1-2200050Bfornotrecordingthereading,inaccordancewiththerequirementsofAppendixDofAPNo.0010120.3)AnalternateinstrumentforrecordingdatapointCoolingWaterOutletTemperature'FwasidentifiedtotheSNPObytheUnit1controlroomat1030onJune6,1996.Analternatedatapointwasrecorded,andindicatedassuchonDataSheet2,forthebalanceofthe24hoursurveillancetest.FPLhasconcludedthattheSNPOdidnotviolatetherequirementsofTechnicalSpecification6.8.1.a,QX5-PR/PSL-1,orOPNo.1-2200050B.
